ImmunizationRecord.org helps people find official information about requesting vaccination records in the United States. We explain where records may be held, link to government and provider resources, and describe the next steps in plain language.

Important: We are not a government agency, immunization registry, healthcare provider, pharmacy, or record-retrieval service. We cannot access, issue, change, or store anyone’s vaccination record.

Why this site exists

Immunization records are managed through a mix of state and local immunization information systems, healthcare providers, pharmacies, schools, and other organizations. The correct route depends on where a vaccination was given, the person’s age, and the reason the record is needed. This site organizes those routes so readers can start with the appropriate official source.

There is no single federal database that gives every person a complete vaccination history. A state registry may also have an incomplete record, especially when vaccinations were given in another state or were never reported to that registry. Our guides explain these limits and give readers additional places to check.

What we publish

Our directory includes dedicated state guides where we have published one. When a dedicated guide is not yet available, we link readers to the CDC’s current state and local record contacts directory.

How content is prepared

We begin with primary sources whenever they are available, including state health department pages, official immunization registry pages, CDC resources, official forms, and provider or pharmacy websites. We compare the wording in a draft with those sources and include direct links so readers can confirm the current requirements themselves.

The site’s operator or editor reviews content before publication for clarity, source support, and obvious inconsistencies. A page may be updated when an official process changes, a link stops working, or a reader reports a possible error. We do not claim that every phone number is called, that every guide receives a multi-person review, or that every page is rechecked on a fixed quarterly schedule.

Who writes the guides

Pages published under the “Editorial Team” byline are prepared for ImmunizationRecord.org and reviewed by the site’s publisher or editor. The byline identifies the publication responsible for the page; it does not mean that the author is a doctor, lawyer, government employee, or representative of the organization being discussed.

We summarize public information and explain administrative steps. We do not diagnose medical conditions, recommend vaccines for an individual, interpret a person’s clinical history, or provide legal advice. Questions about personal healthcare should go to a licensed healthcare professional. Questions about an official record should go to the registry, provider, pharmacy, school, or agency that holds it.

Independence and funding

The site is independently operated and is not affiliated with the CDC, any state or local health department, any immunization information system, or any pharmacy or healthcare provider. The site may display advertising to support its operation. Advertising does not change which official sources we link to or the wording of our guides.

Readers do not pay to use this website. A government agency, provider, pharmacy, or records custodian may have its own rules or fees, which should be confirmed on the official website before submitting a request.

Privacy and safety

You do not need an account to read our guides. Do not send vaccination cards, medical records, Social Security numbers, photo identification, insurance information, or other sensitive health information to this website. Use the secure method specified by the official organization handling your request.
