How to access Washington immunization records in MyIR
Prepare the identity details most likely held by WAIIS
How child and adult accounts work
- Register and sign in to the parent or guardian account.
- Select the option to add a child.
- Enter the child’s legal name and date of birth.
- Use telephone numbers connected with the family’s provider records.
- Review and download each child’s record separately.
- Create a separate MyIR account.
- Use that adult’s own email and legal identity details.
- Try telephone numbers held by their current or former providers.
- Use the signed state form when matching fails.
- Do not attempt to keep an adult child linked as a minor.
What to do when MyIR cannot find a match

The MyIR code goes to an old telephone number
MyIR can send the verification code only to a number already connected with the matching registry record. It cannot redirect the code to an unrelated number.
| Problem | Likely reason | What to do |
|---|---|---|
| No code is offered | None of the entered numbers matched WAIIS. | Add another former household, parent, provider-profile or pharmacy number. |
| Code goes to an old number | The registry still holds outdated contact information. | Ask the provider or Washington DOH to update WAIIS demographics. |
| Text message is blocked | The device blocks outside or automated text messages. | Allow automated texts or select an automated telephone call. |
| Automated call is blocked | Unknown callers are filtered. | Temporarily allow unknown calls or select text delivery. |
| Code is rejected | An older code was entered after a new code was requested. | Restart verification and enter only the newest code. |
| Old number cannot be accessed | Automated identity verification cannot be completed. | Use provider correction or the signed DOH release form. |
Ask a provider, pharmacy or school for the complete record
| Source | Ask for | Use this source when | Watch for |
|---|---|---|---|
| Healthcare provider | A complete WAIIS history and the provider’s clinical immunization record. | Identity correction, missing doses or recent vaccinations. | An ordinary portal summary may show only that clinic’s vaccinations. |
| Pharmacy | The pharmacy history and confirmation that each dose was reported to WAIIS. | Flu, COVID-19, shingles, RSV or other pharmacy-administered vaccines. | A chain account may not include another pharmacy’s vaccinations. |
| Public school | A validated CIS or complete WAIIS printout. | Current student enrollment and school compliance. | Request ahead because access depends on staffing. |
| Private school | Whether staff have WAIIS access and can print a CIS. | A child currently or recently enrolled there. | Not every private school has registry access. |
Request Washington records directly from DOH
Use the Authorization to Release Immunization Records when MyIR does not link, the registered telephone number is unavailable or the record should be delivered by email, fax or postal mail.

How to obtain a medically verified Washington CIS
Washington schools and child-care programs use the Certificate of Immunization Status to document required vaccination or qualifying immunity.
| CIS source | Verification status | Practical action |
|---|---|---|
| Validated CIS printed from WAIIS | Vaccination dates are medically verified by the registry. | Ask a provider, school or eligible child-care program to print it. |
| CIS printed through MyIR | The vaccination dates come from WAIIS and are medically verified. | Download and print after linking the child’s record. |
| Hardcopy CIS signed by a provider | The provider’s signature validates the entered vaccination information. | Take official records to the provider for review and signature. |
| Hardcopy CIS checked by school staff | Staff compare the written dates with attached medical vaccination records. | Complete every date on the CIS and attach the supporting medical records. |
| Another state’s registry record | An official state or territorial registry record is medically verified. | Provide the complete official record to the school or Washington provider. |
| Foreign medical record | Must be an official record with an appropriate clinic or provider identifier. | Provide translation when requested and ask a provider to review it. |
Details many families miss
Washington says the MyIR CIS is medically verified and does not need another signature. However, it can print vaccination dates that do not meet the required minimum ages or intervals, so school staff must review the dates.
A validated CIS can display qualifying laboratory immunity and provider-verified chickenpox disease history already entered into WAIIS.
When a MyIR CIS appears incomplete despite titer or disease-history documentation, ask a provider or school for a validated CIS directly from WAIIS.
The immunization dates must be entered on the hardcopy CIS and then medically verified by a provider or authorized staff comparing attached records.

How to correct a WAIIS or MyIR record
MyIR displays information from the state registry; it cannot clinically edit a vaccine. Ask the organization that administered or documented the vaccination to correct WAIIS first.
| Information | Possible source | Why it helps |
|---|---|---|
| Vaccine name | Provider portal, pharmacy history, vaccine card or clinical summary. | Identifies the product that should appear. |
| Administration date | Appointment history, invoice, insurance claim or clinical record. | Helps locate the original encounter. |
| Administering organization | Clinic, pharmacy, school event, employer or public-health site. | Identifies who should verify and report the dose. |
| Manufacturer and lot | Clinical administration or pharmacy record. | Supports an accurate clinical correction. |
| Former identity details | Old insurance card, portal profile or appointment confirmation. | Helps locate a duplicate or mismatched profile. |
Prepare the current details and any supporting identity information requested by DOH.
Use this route after the provider cannot correct the record or lacks the complete history.
How to add historical vaccines to Washington records
Washington’s 2026 WAIIS guidance says historical vaccination entries must be based on medically verified documentation reviewed using clinical judgment.
| Document | Can support WAIIS historical entry? | Important requirement |
|---|---|---|
| Provider, clinic or hospital EHR record | Yes | Should show an official logo, header, stamp or provider signature. |
| Another state or territory IIS record | Yes | Provide the complete official registry result. |
| Official lifetime record | Yes | Needs a unique healthcare-provider stamp or provider signature. |
| Immigration or foreign medical record | Yes | Needs a clinic or provider stamp, logo or signature and may require translation. |
| Home list or baby book | No | A family-created list is not medically verified. |
| Oral or unsupported written history | No | Vaccination memory without medical proof should not be entered. |
| Unsigned personal lifetime record | No | It must be completed and verified by an appropriate healthcare source. |
| School database printout | Not by itself | Use the underlying medically verified record rather than a local school-system summary. |
Minimum information the historical document should show
Why an adult Washington record may be incomplete
WAIIS is a lifetime registry, but Washington DOH warns that the system does not contain a complete immunization history for every person.
| Situation | Check first | Request |
|---|---|---|
| Pharmacy-administered vaccines | Pharmacy app, account and administering location. | Complete pharmacy history and confirmation of WAIIS reporting. |
| Healthcare employment | Employee health, occupational health, provider and pharmacy. | The exact proof format required by the employer. |
| College or clinical program | Student health office, former school and provider. | Program-specific vaccine and immunity documentation. |
| Military vaccination | Military medical record, TRICARE or VA system. | A complete service immunization history. |
| Old childhood vaccination | Parents, pediatrician, former school and relevant state registry. | Provider-verified or registry documentation. |
| Vaccination outside Washington | The provider, pharmacy or registry where the dose was given. | Official records suitable for provider review and historical entry. |

WA immunization records FAQs
How do I access my Washington immunization records online?
Open MyIR Mobile and register using identity and telephone information that matches WAIIS. After verification, open the Immunizations page and download the available record or school CIS.
Is the WAIIS login intended for regular Washington residents?
No. WAIIS is primarily the secure system used by authorized healthcare providers, schools and public-health organizations. Residents normally use MyIR, a provider, a school or the Washington DOH release form.
How many phone numbers can I try in MyIR?
MyIR allows up to four possible telephone numbers during record linking. Only one number must match the registry, and text or automated-call delivery can be selected for each number.
What if the MyIR verification code goes to an old number?
MyIR cannot redirect the code to a number that is not stored in WAIIS. Ask a healthcare provider or Washington DOH to update the registry demographics, or use the signed record-release form.
Can I add my adult child to my MyIR account?
No. A parent or guardian can add a child younger than 18. A person age 18 or older must create a separate MyIR account.
How long does a Washington DOH record request take?
The current release form says records are usually sent within three to five business days and no later than 15 business days after the signed authorization is received.
Can I electronically sign the Washington release form?
No. The current form states that an electronic signature is not acceptable. Print it, sign it by hand and then send a clear copy by email, fax or postal mail.
Does a CIS printed through MyIR need a provider signature?
No. Washington considers the vaccination dates on a CIS printed through MyIR to be medically verified by WAIIS. School staff may still need to review whether the dates meet minimum age and interval requirements.
Why is a vaccine missing from MyIR?
The provider may not have reported it, the record may use different identity information, a duplicate profile may exist or the vaccination may have been given outside Washington. Contact the administering organization first.
Is WA Verify the same as a complete immunization history?
No. WA Verify is designed for digital COVID-19 vaccination verification and a SMART Health Card. Use MyIR or another official WAIIS access route for a broader immunization history.
