How child and adult accounts work

  • Use telephone numbers connected with the family’s provider records.
  • Review and download each child’s record separately.
Person age 18 or older
  • Create a separate MyIR account.
  • Use that adult’s own email and legal identity details.
  • Try telephone numbers held by their current or former providers.
  • Use the signed state form when matching fails.
  • Do not attempt to keep an adult child linked as a minor.
Important profile distinction: Changing a phone number on the MyIR profile does not change the number stored in WAIIS. A healthcare provider or Washington DOH must update the registry record.
Portal troubleshooting

What to do when MyIR cannot find a match

Name mismatch Try a maiden name, previous surname, suffix, compound surname, hyphen variation or spelling used by the provider.
Telephone mismatch Enter up to four possible numbers. Only one number needs to match the registry.
Address mismatch Try the address used during vaccination or the address held by the primary-care office.
Child does not link Check the child’s exact legal name, birth date and parent telephone information recorded by the pediatrician.
Duplicate profile The record may be divided between current and former names, addresses, telephone numbers or an incorrect birth date.
No Washington record The vaccinations may have been administered outside Washington or never reported to WAIIS.

Verification-code help

The MyIR code goes to an old telephone number

MyIR can send the verification code only to a number already connected with the matching registry record. It cannot redirect the code to an unrelated number.

Email privacy warning: The current form says the record will not remain encrypted when sent by ordinary email. When requesting delivery to a third party, confirm how that organization protects and shares the information.
Telephone limitation: Washington staff may ask identifying questions and confirm whether a record exists, but vaccination details are not released over the telephone.
Authorization duration: For a child request, the form states that authorization expires when the child turns 18 or 18 years after signing, whichever comes first. It can be revoked in writing by mail or fax, but revocation does not affect information already released.
School and child-care records

How to obtain a medically verified Washington CIS

Washington schools and child-care programs use the Certificate of Immunization Status to document required vaccination or qualifying immunity.

Details many families miss

MyIR CIS is accepted but still reviewed

Washington says the MyIR CIS is medically verified and does not need another signature. However, it can print vaccination dates that do not meet the required minimum ages or intervals, so school staff must review the dates.

Validated WAIIS CIS may show immunity evidence

A validated CIS can display qualifying laboratory immunity and provider-verified chickenpox disease history already entered into WAIIS.

MyIR may not display every immunity detail

When a MyIR CIS appears incomplete despite titer or disease-history documentation, ask a provider or school for a validated CIS directly from WAIIS.

A blank CIS with records attached is not enough

The immunization dates must be entered on the hardcopy CIS and then medically verified by a provider or authorized staff comparing attached records.

Direct provider delivery: Washington guidance says a healthcare provider may send a medically verified CIS directly to the school or child-care program with the parent or guardian’s agreement.
Paper-copy shortcut: A school actively using the state School and Child Care Immunization Module may not require a separate paper CIS when the student’s status is complete in the system. Ask the school before requesting another copy.
Child-care renewal difference: Washington guidance says children attending child care must provide an updated medically verified CIS annually. Confirm the current local deadline with the program.
Conditional attendance

What “Conditional” means on the Washington CIS

Receive every dose currently eligible to be administered Conditional status is used while waiting for the next medically valid dose date in a vaccine series.
Submit the current CIS and supporting proof The school or child-care program needs documentation of the doses already received.
Parent or guardian signs the conditional-status section The signature acknowledges the follow-up rules and timeline.

Missing or incorrect information

How to correct a WAIIS or MyIR record

MyIR displays information from the state registry; it cannot clinically edit a vaccine. Ask the organization that administered or documented the vaccination to correct WAIIS first.

Wrong name or birth date Ask the provider to correct the demographic record or use Washington’s official IIS record-update contact.
Missing pharmacy vaccination Ask the pharmacy to check its administration record and confirm whether the dose was submitted to WAIIS.
Duplicate patient profiles Ask the provider to search former names, addresses, phone numbers and possible birth-date errors before creating another record.
Incorrect vaccine date Ask the administering organization to compare the original appointment, lot and clinical record before correcting the entry.
Recent dose has not appeared Refresh MyIR, then contact the administering provider if the dose remains missing.

Why an adult Washington record may be incomplete

WAIIS is a lifetime registry, but Washington DOH warns that the system does not contain a complete immunization history for every person.

How do I access my Washington immunization records online?

Open MyIR Mobile and register using identity and telephone information that matches WAIIS. After verification, open the Immunizations page and download the available record or school CIS.

Is the WAIIS login intended for regular Washington residents?

No. WAIIS is primarily the secure system used by authorized healthcare providers, schools and public-health organizations. Residents normally use MyIR, a provider, a school or the Washington DOH release form.

How many phone numbers can I try in MyIR?

MyIR allows up to four possible telephone numbers during record linking. Only one number must match the registry, and text or automated-call delivery can be selected for each number.

What if the MyIR verification code goes to an old number?

MyIR cannot redirect the code to a number that is not stored in WAIIS. Ask a healthcare provider or Washington DOH to update the registry demographics, or use the signed record-release form.

Can I add my adult child to my MyIR account?

No. A parent or guardian can add a child younger than 18. A person age 18 or older must create a separate MyIR account.

How long does a Washington DOH record request take?

The current release form says records are usually sent within three to five business days and no later than 15 business days after the signed authorization is received.

Can I electronically sign the Washington release form?

No. The current form states that an electronic signature is not acceptable. Print it, sign it by hand and then send a clear copy by email, fax or postal mail.

Does a CIS printed through MyIR need a provider signature?

No. Washington considers the vaccination dates on a CIS printed through MyIR to be medically verified by WAIIS. School staff may still need to review whether the dates meet minimum age and interval requirements.

Why is a vaccine missing from MyIR?

The provider may not have reported it, the record may use different identity information, a duplicate profile may exist or the vaccination may have been given outside Washington. Contact the administering organization first.

Is WA Verify the same as a complete immunization history?

No. WA Verify is designed for digital COVID-19 vaccination verification and a SMART Health Card. Use MyIR or another official WAIIS access route for a broader immunization history.
