California now requires healthcare providers that administer immunizations to report those administrations to a California immunization registry. For most counties that registry is CAIR; providers in the eight Healthy Futures/RIDE counties use that regional system.
Vaccines from 2023 onward
California’s broader reporting mandate makes recent registry histories more likely to contain vaccines administered by participating California providers.
Vaccines before 2023
Older doses can be missing because broad all-provider reporting was not always required.
Very old records
CAIR itself did not begin receiving records until the late 1990s and early 2000s, so older childhood history may exist only outside the registry.
Important historical nuance:
Before January 1, 2023, California’s reporting rules were narrower. CDPH notes that COVID vaccinations and pharmacy-administered vaccinations had separate reporting requirements before the broader mandate.
Bottom line:
An official DVR is valuable, but an empty or incomplete DVR does not automatically mean a vaccination never happened.

Open the official CDPH Digital Vaccine Record request page
Make sure the page is on the myvaccinerecord.cdph.ca.gov domain before entering private information.
Select the record type
Choose “I want my California Immunization Record” when you need available routine childhood or adult vaccination history.
Enter the record owner’s first and last name
Start with the identity that healthcare providers most likely used when vaccinations were reported.
Enter the date of birth
Use the exact month, day and year.
Provide a current mobile number or email
The official request page recommends trying email if a current mobile number does not produce a match.
Create a private four-digit PIN
You will need the same PIN to unlock the secure record link.
Confirm your authority to access the record
The portal requires you to certify that you are the patient or the patient’s parent/guardian.
Submit the request
When the submitted information matches the immunization registry, California sends a secure link to the associated phone number or email.
Open the secure link within 24 hours

Family-account problem:
A pharmacy or clinic appointment may have been scheduled under a parent’s, spouse’s or household phone/email. The contact used during the vaccination can matter more than the contact you use today.

Parents and guardians

Parents and legal guardians can request available records for minor children when the registry can match the child’s information and the connected contact details.
Current multi-child rule
California Immunization Record:
CDPH’s current FAQ says a parent or legal guardian can request DVRs for up to three minor dependents in one submission when all records are linked to the same cellphone number or email address.
But follow the live request form:
The current request page also displays wording telling parents with multiple records connected to one contact to enter each DVR request separately. Because these official instructions are not perfectly aligned, follow the workflow shown by the live portal during your request.
COVID QR requests are different.
The FAQ says the multi-dependent feature applies to California Immunization Records and not to COVID-19 Vaccine Records with QR codes.

Check legitimate former names
Older vaccinations may be connected with a maiden name, previous surname or different spelling.
Check prior contact details
Ask the vaccinating provider which phone number or email is stored in the patient’s record.
Confirm that the provider reported the vaccination
A clinical record can exist even when the dose was never successfully added to the registry.
Check Healthy Futures/RIDE when relevant
This matters when vaccinations were administered by providers in one of its eight counties.

Duplicate-record warning:
California’s duplicate-patient merge process is provider-facing. CDPH warns providers that a merge cannot be reversed, so they must make sure both records truly belong to the same person.
DVR status labels
What does “Invalid” mean beside a vaccine dose?
The DVR can label a vaccination as “Invalid” when the registry’s timing/spacing logic determines that the administered dose did not align with the immunization schedule used by CAIR.
Use the information icon:
CDPH says the DVR’s information bubble can explain why the dose is being treated as invalid based on the historical record and schedule logic.
Invalid in DVR does not automatically mean rejected everywhere.
CDPH notes that a dose marked invalid by the DVR may still satisfy a school, employer or other organization’s requirements. Ask the receiving organization and an appropriate healthcare professional rather than making the decision yourself.
Do not manually alter the PDF.

Use the DVR for school proof—but know what the school actually reviews
CDPH describes the Digital Vaccine Record as an official California record that can be used for school or child-care entry. School personnel still review the documentation against California’s current admission and advancement requirements.

Parents do not self-certify the Blue Card.
Provide the DVR, healthcare-provider record, prior-school record or other acceptable evidence and allow authorized staff to determine the student’s status.

What happens when the school record is incomplete?
Conditional admission
A student who has started all required vaccine series but is not currently due for another dose may qualify for conditional admission under California’s schedule.
30-day review
Schools must review conditionally admitted students’ immunization records at least every 30 days until requirements are completed or an applicable exemption is filed.
Overdue later
Current implementation guidance says a school discovering an admitted student is overdue must notify the parent and allow no more than 10 school days for required vaccination or an applicable medical exemption before exclusion.
Homeless and foster transfer students
Immediate enrollment protection:
California says qualifying homeless and foster students transferring into a new school may be enrolled immediately even when immunization records are unavailable, with up to 30 school days allowed while records are located.
This does not eliminate immunization requirements.
The school still works to obtain the student’s records and bring the file into compliance.
Medical exemption route
A lost record and a California medical exemption are not the same thing
California uses CAIR-ME for new school and child-care medical exemptions. Parents/guardians begin the process in CAIR-ME and a California-licensed physician issues and manages the exemption when medically appropriate.
Missing record
Recover documentation from DVR, provider, pharmacy, school, Healthy Futures or another state.
Medical exemption
Use CAIR-ME only when a qualifying medical reason is being documented through the current physician process.
Do not use CAIR-ME simply because you cannot find a vaccine record.
Record recovery should be attempted first unless an actual medical-exemption situation exists.

Open the account used for the vaccination
Check the pharmacy app, healthcare portal or patient portal connected with the appointment.

What happens when you lock or unlock a CAIR record?
CAIR is an opt-out sharing registry. California requires reporting of immunization information, while patients and parents have a separate right to limit sharing of the CAIR record with other participating organizations.

Limits access by other CAIR organizations while allowing the organization responsible for the record—and applicable public-health access—to continue handling it under CAIR rules.

Restores sharing of the record with other authorized CAIR users.
Locking does not delete the record.

Why are older vaccines missing from my California record?

What if my vaccinations were given outside California?
Request the official record from the state registry, provider or pharmacy where the vaccination occurred. Give verified documentation to your California healthcare provider if you want the provider to review whether historical doses can be submitted to CAIR.
