Open the official CIIS Public Portal Confirm the address is on the state.co.us domain before entering private information.
Choose “Me” or “Dependent” Select “Me” for your own record. Choose “Dependent” when requesting the record of a minor or legal dependent you are authorized to access.

How to get a child’s Colorado immunization record

Choose the dependent option Use it only when you are authorized to request the child’s or legal dependent’s record.

Colorado school immunization records for 2026–27

Seventh-grade Tdap change: Beginning with the 2026–27 school year, Colorado requires the adolescent Tdap dose before the first day of seventh grade rather than at sixth-grade entry.

What to do after a CIIS opt-out

Colorado allows an individual or parent or legal guardian to exclude immunization information from CIIS. A person who opts out is responsible for maintaining their own or their child’s records.

  • Store paper and digital backups.
  • Do not expect the public portal to rebuild the excluded history.
