What to do after a CIIS opt-out
Colorado allows an individual or parent or legal guardian to exclude immunization information from CIIS. A person who opts out is responsible for maintaining their own or their child’s records.

No. A CIIS record contains vaccinations that providers, pharmacies and some schools reported and entered. Older, out-of-state, military, federal or paper-only records may be missing.
What can I download from the CIIS Public Portal?
When available, the portal can provide a full immunization record, school certificate, COVID-19 record or COVID-19 QR code. Choose the document required by the receiving organization.
What should I do when the CIIS portal does not work?
Check the identity and contact information, enable JavaScript and cookies if a validation page appears, then use the official Immunization Record Information Request Form or manual release-form route if access still fails.
How soon should a new vaccination appear in CIIS?
Colorado’s myColorado guidance says to allow approximately 24 to 72 hours for a newly reported vaccination to appear. Contact the administering provider if it remains missing.
Can I store a CIIS record in myColorado?
Yes. Download the current PDF from CIIS and upload it manually to myCards and Documents. Uploaded documents do not update automatically when new vaccinations are reported.
What records are accepted for Colorado school attendance?
A school may require an immunization record showing the student is up to date, an in-process plan signed by a healthcare provider, a medical or nonmedical exemption certificate, or a combination of these documents.
What if vaccinations were administered outside Colorado?
