Colorado Immunization Records Online: CIIS Steps 2026

Colorado CIIS guide · Online retrieval, corrections and school-ready proof

Retrieve the Right Colorado Vaccine Record

Colorado residents can start with the official CIIS Public Portal to view and print an available record for themselves or a legal dependent. The portal works only when the identity and contact information entered match the patient profile stored in CIIS.

Follow the steps below to avoid temporary lockouts, choose the correct PDF, retrieve a child’s record, correct a missing vaccination, meet a school deadline or use CDPHE’s secure manual-release route.

Protect your medical information: This independent guide cannot search CIIS or retrieve a record. Enter names, birth dates, access codes, photo identification and supporting documents only after opening the official Colorado portal, CDPHE form or your healthcare provider’s secure system.

Official online workflow

How to get Colorado immunization records online

Open the official CIIS Public Portal Check the address before entering private information. The resident portal begins at copublicportal.state.co.us.
Choose “Me” or “Dependent” Select your own record or a minor legal dependent’s record. Do not use the dependent route for another adult.
Enter the patient identity exactly Use the first name, last name, date of birth and gender documented by the healthcare provider.
Select the correct relationship Parents or guardians should choose the relationship that accurately describes their legal connection to the minor.
Choose text or email verification Enter a mobile phone number or email already stored in the patient’s CIIS profile.
Request the access code once Keep the page open and check text messages, email, spam, junk and filtered folders before requesting another code.
Enter the verification code carefully Avoid guessing. Two incorrect code entries can trigger a 30-minute lockout.
Review the vaccination history Compare the vaccine names and administration dates with pharmacy, provider and paper records.
Download the correct document Choose the full record for broad history or the school certificate when the receiving school requests that format.
Save a secure backup Keep a private PDF and one printed copy. Do not post the document, secure link or QR code publicly.
Browser requirement: The public-portal instructions linked by CDPHE identify Google Chrome, Microsoft Edge and Mozilla Firefox as supported browsers.
Avoid a failed search

Which details must match the CIIS patient profile?

Exact-match fields and practical fixes
Portal field Common mismatch What to check
First and last name Former surname, maiden name, compound surname, suffix, spacing or typing difference. Use only legitimate name versions that may appear in the provider’s patient profile.
Date of birth Incorrect date or data-entry error. Compare the date in the provider portal and ask the clinic to verify its CIIS profile.
Gender The entered value differs from the existing registry profile. Ask the provider to review and appropriately correct the demographic profile.
Mobile phone Old number, parent number, landline, missing number or inability to receive SMS. Try the mobile number used during the vaccination visit.
Email address Old personal, family, work or school email. Try each legitimate address that may have been supplied to the provider.
Dependent relationship The selected relationship is incorrect or staff review is required. Choose the accurate relationship and use the manual route if supporting documents are required.
Thirty-minute lockout risk: Three unsuccessful record searches can trigger a 30-minute lockout. Two incorrect verification-code entries can also trigger a 30-minute lockout. Review every field rather than repeating the same unsuccessful request.
After a provider updates your contact details: Colorado’s myColorado guidance says the change takes at least 24 hours to reflect. Wait before trying the portal again.
Choose the right proof

Which CIIS document should you download?

Available CIIS record formats explained
Download Best use Check before sending
Full immunization record Personal history, healthcare review, college, employment or a request needing routine vaccines. Confirm all expected vaccines and administration dates appear.
School certificate Colorado K–12, child care or another education program requesting school-formatted proof. Ask the school whether it accepts the CIIS certificate without another local form.
COVID-19 record A separate COVID-19 document when that specific proof is requested. Use the full record when other vaccinations are also required.
COVID-19 QR code Digital verification when the receiving organization supports the QR format. Do not publish, forward widely or display the code unnecessarily.
A screenshot is not the best final document. Download the complete PDF, confirm all pages open correctly and ask the receiving organization which file it accepts.
Portal troubleshooting

No exact CIIS match? Follow this order

Name mismatch Try the former surname, maiden name, hyphenated name or spelling actually used by the healthcare provider.
Old phone or email Try legitimate contact details used when the vaccination appointment was registered.
Contact details missing Ask the provider to add or correct the mobile phone number or email in CIIS.
Vaccination not reported Contact the clinic, pharmacy, school clinic or public-health office that administered the dose.
Multiple records found Submit the information-request form so the CIIS Help Desk can review possible duplicate profiles.
Vaccinated outside Colorado Contact the other state’s registry or the provider that administered the vaccine.
Older paper-only history Search former providers, schools, military files, family records and employer health offices.
Previous CIIS opt-out Follow the official rescind-opt-out process and retain personal documentation while the record is rebuilt.
Stop repeating the same unsuccessful search A repeated mismatch can create a temporary lockout without correcting the underlying record.
Ask the provider to verify demographics Confirm the patient’s exact name, former name, birth date, gender, phone and email.
Ask whether each dose was accepted by CIIS Request the vaccine name, administration date and confirmation of successful reporting.
Submit the official CIIS information-request form Use this for no exact match, multiple records, an incorrect record or an unresolved portal problem.
Use the notarized release form when needed Prepare identification and any guardianship or supporting documentation before submitting it.
Submit only one help form. CDPHE’s official information-request form warns that submitting several forms increases the response delay.

How to complete the CDPHE record-release form

The Request to Release Immunization Record Form currently linked by CDPHE is a formal authorization. It is different from the shorter online troubleshooting form.

Information and documents to prepare

Patient’s full name
Date of birth
Complete mailing address
Phone number and email
Reason for the request
Patient, parent or guardian signature
Child’s name for a dependent request
Proof of parent, guardian or legal-custody authority when applicable
Legible copy of a driver license or another secure, verifiable ID
Notary signature, commission information and seal

Submission workflow

Download the form from CDPHE’s official records page Confirm you are using the form currently linked by the department.
Complete all patient and contact fields Incomplete forms are not accepted.
Select and explain the reason for the request The form lists medical care, legal reasons, school, insurance and other purposes.
Sign the correct authorization section Use the patient section for your own record or the child-record section when acting as an authorized parent or guardian.
Have the signature notarized Do not send an unsigned or unnotarized form.
Attach identification and supporting documentation Include a readable copy, not the original identification document.
Follow the current secure-email or fax instructions CDPHE says approved records are securely transmitted to the requester through secure email or fax.
Keep a private copy Save the completed form, identification copy and submission confirmation.
Check the form version before submission: The form currently linked by CDPHE carries an older revision date. Follow the current instructions on CDPHE’s live records page and confirm the submission method if anything conflicts.
Correction workflow

What to do when a vaccination is missing or incorrect

Begin with the provider, pharmacy or clinic that administered the vaccination. The administering organization has the clinical documentation needed to verify a dose.

Details to collect before requesting a correction
Detail Where to find it Why it helps
Vaccine name Provider portal, pharmacy record, receipt or paper card. Identifies the clinical entry that should appear.
Administration date Appointment history, claim, receipt or visit summary. Allows staff to locate the original encounter.
Administering location Clinic, pharmacy, school event or public-health office. Shows which organization must verify the dose.
Manufacturer and lot number Clinical administration or pharmacy record. Adds clinical verification when available.
Identity used during the visit Appointment confirmation, portal profile or insurance claim. May reveal a duplicate or mismatched patient profile.
Provider correction script “My CIIS record is missing the [vaccine] administered at your location on approximately [date]. Please verify the administration record, the patient identity and contact information used, and whether the dose was successfully accepted by CIIS or attached to another profile.”
Allow reporting time: Colorado’s current myColorado guidance says a newly administered vaccination may take approximately 24–72 hours to appear.
Do not invent or estimate vaccination dates. Use verified documentation. Ask the school, employer or healthcare professional what alternative evidence is accepted when an original record cannot be found.
Parent and guardian access

How to retrieve a child’s Colorado immunization record

Select the dependent request Use this route only when you are authorized to request the minor child’s record.
Enter the child’s exact identity information Use the name, birth date and gender recorded by the pediatrician or vaccinating provider.
Use the contact information stored in the child’s profile The verification number or email may be the parent contact used during the vaccination visit.
Select the accurate relationship Choose the parent or guardian relationship that applies.
Search each child separately Siblings may share a parent contact but still have separate profiles and different missing-record issues.
Use staff assistance when documents require review A guardian or legal custodian may need supporting documentation with the manual release form.
Pediatrician call script “I cannot retrieve my child’s CIIS record. Please confirm the child’s exact name, date of birth and gender in CIIS, the parent phone number or email attached to the profile, and whether every vaccination was successfully reported.”
K–12, child care, camp and college

How to prepare Colorado school immunization proof

Ask the school nurse, child care administrator, registrar or student-health office for the exact missing dose, acceptable document and deadline before repeating a vaccination or ordering laboratory testing.

Best record route by education situation
Situation Likely document Practical action
K–12 enrollment CIIS school certificate, provider record, in-process plan or valid exemption documentation. Ask the school nurse which vaccine or dose is missing.
Child care or preschool Age-appropriate vaccine history or other accepted Colorado documentation. Confirm the program’s required form and submission date.
Camp Record, in-process plan or exemption information required by the program. Give the required documentation to each separate camp.
College or university Campus health form, CIIS history or provider documentation. Check the student-health portal before uploading a general PDF.
Healthcare training Program-specific vaccine dates, laboratory evidence or occupational-health form. Get the written requirement list before ordering tests.
Transferred from another state Previous state registry or provider record. Provide the complete history for Colorado requirement review.
2026–27 seventh-grade Tdap change: Colorado requires one Tdap dose before the first day of seventh grade beginning with the 2026–2027 school year.

When the family may not need further Tdap action

The school already has an adolescent Tdap dose given on or after the student’s 10th birthday
The school has a provider-signed in-process plan showing the student is catching up
The school has a complete medical or nonmedical Tdap exemption certificate
The school confirms its existing documentation already satisfies the new timing rule
Deadline rescue plan

What to do when the record deadline is close

Deadline is today

Call the receiving office and the provider. Ask which exact dose is missing and whether a portal PDF, provider fax or temporary documentation is accepted.

Deadline is within 2–3 days

Use the CIIS portal immediately, contact the administering provider for missing doses and submit one CIIS information-request form if access fails.

You have one week or longer

Collect provider, pharmacy, school and out-of-state records, then ask the receiving organization to review them before the final due date.

A portal request does not guarantee a complete record. When a deadline is close, contact the original provider and receiving organization at the same time rather than waiting on one route.

Downloaded file not visible?

Android

Check the Downloads folder. Some devices may require a PDF reader before the downloaded record can be opened.

iPhone or iPad

Check the Files app and iCloud Drive. Confirm that the device is signed into iCloud when the file is not visible.

Older and out-of-state history

How to rebuild an incomplete Colorado record

Download the available CIIS record Use it as a starting list rather than assuming it is a complete lifetime history.
Contact previous doctors and pediatricians Request the complete immunization section of the medical record.
Check every pharmacy used for adult vaccinations Try the pharmacy account, old phone number, former email and exact store location.
Contact previous schools and colleges Ask the health office for immunization documentation, not only an academic transcript.
Check military, VA and occupational-health records These records can remain outside CIIS.
Contact the state where each vaccine was administered There is no universal national resident portal that automatically combines every state.
Bring verified outside records to a Colorado provider Ask whether the provider can reconcile or submit the historical vaccinations to CIIS.
Interstate exchange may help, but it is not guaranteed: CDPHE says CIIS exchanges some information with Arizona, Kansas, Nevada, New Mexico, Oklahoma, Pennsylvania and Wyoming. Still contact the original state when a dose is missing.
Keep the original: Scan or photograph an old vaccination card before giving a copy to a school, employer or healthcare office.
Important distinction

CIIS opt-out is not a school vaccine exemption

CIIS opt-out

Uses a registry procedure to exclude or remove the person’s immunization information from CIIS.

School exemption

Uses Colorado’s separate medical or nonmedical exemption rules for school or licensed child care attendance.

Download and preserve records before opting out. After registry information is removed, the individual or family becomes responsible for maintaining usable copies.

Common questions

Colorado immunization records FAQs

How do I get my Colorado immunization record online?

Use the official CIIS Public Portal. Choose whether the request is for you or a legal dependent, enter identity and contact information that exactly matches the CIIS profile, verify the code sent by text or email, and download the available record.

What information must match the CIIS profile?

The first name, last name, date of birth, gender, phone number or email must match what is stored in CIIS. A former surname, old phone number, missing contact information or data-entry difference can prevent a match.

Can a parent retrieve a child’s Colorado vaccine record?

A parent or guardian can request an available record for a minor dependent. The child’s identity details and the phone number or email used for verification must match the child’s CIIS profile.

Why does the Colorado portal say no exact match?

The identity or contact information may be different in CIIS, contact details may be missing, the record may be duplicated, the vaccination may not have been reported, or the dose may have been administered outside Colorado.

Can repeated failed searches lock the CIIS portal?

The portal instructions currently linked by CDPHE say three unsuccessful record searches can cause a 30-minute lockout. Entering the verification code incorrectly twice can also cause a 30-minute lockout.

Which record can I download from CIIS?

The portal may offer a full immunization record, a school certificate, a COVID-19 record and a COVID-19 QR code. Available information depends on what providers have reported to CIIS.

How long does a recent vaccination take to appear?

Colorado’s myColorado guidance says to allow approximately 24 to 72 hours for a recently administered vaccination to appear. Updated contact information takes at least 24 hours to reflect.

What does the CDPHE manual release form require?

The form currently linked by CDPHE requires a complete request, proper identification, applicable supporting documents, signature and notarization. Approved records are transmitted through secure email or fax.

Is opting out of CIIS the same as a school vaccine exemption?

No. CIIS opt-out removes registry information through a separate process. A Colorado medical or nonmedical school exemption follows different rules and documentation requirements.

What if my vaccinations were given outside Colorado?

Contact the registry, provider or pharmacy in the state where each vaccine was administered. A Colorado provider may also be able to review verified outside records or access certain exchanged records.
