How to Get Immunization Records In California Online in 2026

California · Digital Vaccine Record · CAIR · 2026

Request, Open and Fix Your California Immunization Record

California’s Digital Vaccine Record is now much more than a COVID QR-card tool. It can provide an official California Immunization Record containing routine childhood and adult vaccines that healthcare providers and pharmacies reported to the state’s immunization registry.

Start with the public DVR portal—not a CAIR2 provider login. If your record does not match, is incomplete, or contains incorrect information, the correct next step depends on whether the problem is identity matching, a missing clinical dose, an older record, or a vaccination stored in Healthy Futures/RIDE.

Choose the California record task you need to finish

Go directly to the problem you have instead of using generic vaccine-record calculators or state finders.

First decision

Which California immunization-record system should you use?

Most residents Digital Vaccine Record

Use California’s public DVR portal for your own available record or an eligible child’s record.

Choose the California Immunization Record option when you need routine vaccinations, not only COVID-19 proof.

Matching problem My DVR Assistant

Use the Assistant when your record cannot be found, your personal/contact information needs review, or another DVR access problem remains.

Identity verification may use ID.me.

Clinical dose problem Healthcare provider or pharmacy

Use the organization that actually administered a missing or incorrect vaccine.

The CAIR Help Desk does not directly add missing clinical doses to a resident’s record.

Regional registry Healthy Futures / RIDE

This separate California registry covers providers in Alpine, Amador, Calaveras, Mariposa, Merced, San Joaquin, Stanislaus and Tuolumne counties.

Use where the vaccination was administered—not only where you live today.

Do not use CAIR2 as a resident login. CAIR2, CAIR Hub and related roster/account tools are for authorized healthcare, school, child-care and public-health users. Residents normally use the Digital Vaccine Record, My DVR Assistant, a provider or the appropriate registry help route.
Why record completeness varies

The January 1, 2023 reporting change matters

California now requires healthcare providers that administer immunizations to report those administrations to a California immunization registry. For most counties that registry is CAIR; providers in the eight Healthy Futures/RIDE counties use that regional system.

Vaccines from 2023 onward

California’s broader reporting mandate makes recent registry histories more likely to contain vaccines administered by participating California providers.

Vaccines before 2023

Older doses can be missing because broad all-provider reporting was not always required.

Very old records

CAIR itself did not begin receiving records until the late 1990s and early 2000s, so older childhood history may exist only outside the registry.

Important historical nuance: Before January 1, 2023, California’s reporting rules were narrower. CDPH notes that COVID vaccinations and pharmacy-administered vaccinations had separate reporting requirements before the broader mandate.
Bottom line: An official DVR is valuable, but an empty or incomplete DVR does not automatically mean a vaccination never happened.
Official online process

How to get immunization records in California online

Open the official CDPH Digital Vaccine Record request page Make sure the page is on the myvaccinerecord.cdph.ca.gov domain before entering private information.
Select the record type Choose “I want my California Immunization Record” when you need available routine childhood or adult vaccination history.
Enter the record owner’s first and last name Start with the identity that healthcare providers most likely used when vaccinations were reported.
Enter the date of birth Use the exact month, day and year.
Provide a current mobile number or email The official request page recommends trying email if a current mobile number does not produce a match.
Create a private four-digit PIN You will need the same PIN to unlock the secure record link.
Confirm your authority to access the record The portal requires you to certify that you are the patient or the patient’s parent/guardian.
Submit the request When the submitted information matches the immunization registry, California sends a secure link to the associated phone number or email.
Open the secure link within 24 hours Enter the four-digit PIN you created.
Review and save every page Check the patient’s identity, vaccine names, administration dates and whether all expected providers are represented.
Matching checklist

Prepare these details before assuming CAIR has no record

Current legal first name
Current legal last name
Maiden or former surname
Hyphenated name used previously
Exact date of birth
Current mobile number
Previous mobile numbers
Current email
Previous personal email
Former school/work email
Parent contact used for a child
Provider or pharmacy name
Approximate vaccination dates
County where vaccines were given
Other states where vaccinated
Existing paper/provider record
Family-account problem: A pharmacy or clinic appointment may have been scheduled under a parent’s, spouse’s or household phone/email. The contact used during the vaccination can matter more than the contact you use today.
Secure access

Understand the four-digit PIN and 24-hour link

Create the PIN yourself

The DVR request form asks you to create a four-digit PIN. California does not generate this PIN for you.

Use the same PIN later

The PIN is required when opening the secure link sent to the matching phone number or email.

Open within 24 hours

The delivered link expires after the current 24-hour access window.

Forgot the PIN or missed the deadline? Start a new DVR request and create a new PIN. There is no need to guess repeatedly.
iPhone users: If the secure SMS seems missing, check the “Unknown Senders” message area when that filtering option is enabled.
Saved records do not automatically refresh. After another vaccination or a registry correction, return to the DVR website and request a fresh record.
Choose the right proof

California Immunization Record or COVID-19 QR record?

Use the DVR output that matches the receiving organization’s request
Record What it shows Best use
California Immunization Record Available routine childhood/adult vaccines reported to the California registry, plus vaccine-status/recommendation information. School, child care, college, work, healthcare review and personal files.
COVID-19 Vaccine Record COVID vaccine information plus SMART Health Card QR code when available. When a receiving organization specifically asks for COVID digital proof.
Provider / pharmacy record Vaccinations documented by that specific healthcare organization. Missing-dose verification and fast backup documentation.
School Blue Card School-maintained immunization dates and requirement status. California school and child-care recordkeeping.
Language availability: California’s DVR website provides automatic interface translation through Google Translate. CDPH says the saved/printed California Immunization Record PDF itself is available only in English.
Parents and guardians

Request a child’s California immunization record

Parents and legal guardians can request available records for minor children when the registry can match the child’s information and the connected contact details.

Current multi-child rule

California Immunization Record: CDPH’s current FAQ says a parent or legal guardian can request DVRs for up to three minor dependents in one submission when all records are linked to the same cellphone number or email address.
But follow the live request form: The current request page also displays wording telling parents with multiple records connected to one contact to enter each DVR request separately. Because these official instructions are not perfectly aligned, follow the workflow shown by the live portal during your request.
COVID QR requests are different. The FAQ says the multi-dependent feature applies to California Immunization Records and not to COVID-19 Vaccine Records with QR codes.

No record found

Use this order when the DVR cannot find a match

Try email when the phone number does not match The current California request page specifically recommends trying email if your current mobile number produces no match.
Check legitimate former names Older vaccinations may be connected with a maiden name, previous surname or different spelling.
Check prior contact details Ask the vaccinating provider which phone number or email is stored in the patient’s record.
Confirm that the provider reported the vaccination A clinical record can exist even when the dose was never successfully added to the registry.
Check Healthy Futures/RIDE when relevant This matters when vaccinations were administered by providers in one of its eight counties.
Use My DVR Assistant Request official review instead of repeatedly guessing contact information.
No match does not prove that CAIR is empty. The record can exist but fail public matching because identity/contact information differs.
Official DVR review

When My DVR Assistant is the correct next step

Record not located

Use it after reasonable legitimate phone/email and identity checks fail.

Personal information is wrong

Use the Assistant when the DVR contains incorrect identity or contact information.

Official review needed

CDPH says users are generally notified of findings and follow-up actions within about a week.

ID.me verification: My DVR Assistant may ask you to verify your identity through ID.me before completing the review request.
Assistant eligibility: CDPH currently states that you must be a California resident and have received at least one vaccine dose in California to use the My DVR Assistant vaccine-record review service.
Minor request: If the record owner is younger than 18, a parent/legal guardian generally submits the Assistant request unless the person is an emancipated minor age 16 or older.
DVR cannot be located or updated by telephone. CDPH directs these record-review issues to My DVR Assistant.
Missing or incorrect clinical dose

Do not use the wrong office to correct a vaccine dose

California correction routing
Problem Start with Prepare
Missing vaccination Provider/pharmacy that administered it. Vaccine name, date, location and administration record.
Wrong vaccination date Original administering provider. Clinical document containing the correct date.
Wrong contact information My DVR Assistant / provider. Current and previous phone/email.
Wrong or former name My DVR Assistant / provider. Current and former legal names.
Possible duplicate CAIR profiles Provider + CAIR support. Both name/contact versions and providers associated with each history.
Out-of-state vaccine missing Source-state IIS/provider + California provider. Official outside-state record.
CAIR Help Desk cannot add missing doses directly for you. CDPH instructs patients with missing or incorrect vaccination doses to work with the healthcare provider that can verify the clinical record and submit the missing or corrected data.

Provider correction script “My California immunization record is missing the [vaccine] administered by your organization around [date]. Please locate the original administration record, verify the patient identity attached to it, and confirm whether the dose was successfully submitted to CAIR or Healthy Futures/RIDE.”
Duplicate-record warning: California’s duplicate-patient merge process is provider-facing. CDPH warns providers that a merge cannot be reversed, so they must make sure both records truly belong to the same person.
DVR status labels

What does “Invalid” mean beside a vaccine dose?

The DVR can label a vaccination as “Invalid” when the registry’s timing/spacing logic determines that the administered dose did not align with the immunization schedule used by CAIR.

Use the information icon: CDPH says the DVR’s information bubble can explain why the dose is being treated as invalid based on the historical record and schedule logic.
Invalid in DVR does not automatically mean rejected everywhere. CDPH notes that a dose marked invalid by the DVR may still satisfy a school, employer or other organization’s requirements. Ask the receiving organization and an appropriate healthcare professional rather than making the decision yourself.
Do not manually alter the PDF. A clinical or registry correction should come through the healthcare provider or official review route.

Healthy Futures script “I need an immunization record for [myself/my child]. Vaccines were administered in [county] by [provider] around [date]. Please explain the current personal-record request route and what identifying information you need.”
TK–12 school records

Use the DVR for school proof—but know what the school actually reviews

CDPH describes the Digital Vaccine Record as an official California record that can be used for school or child-care entry. School personnel still review the documentation against California’s current admission and advancement requirements.

California TK/K–12 requirement snapshot
Checkpoint Published summary Important qualification
K–12 admission 4 polio, 5 DTaP, 3 Hep B, 2 MMR and 2 varicella. Age, vaccine type and timing rules can allow different valid dose counts.
7th–12th grade admission K–12 admission requirements plus one qualifying Tdap. At least one pertussis-containing dose on or after the seventh birthday is part of the published rule.
7th-grade advancement One Tdap. The prior separate two-dose varicella advancement check expired after June 30, 2025.
Do not treat the summary table as a dose calculator. Minimum ages, spacing, combination vaccines, conditional admission, transfers and medical exemptions can change the student’s final status.

Blue Card / CDPH 286

California schools and pre-kindergarten facilities maintain an immunization record commonly called the Blue Card. School or child-care staff use the documentation supplied by the family or retrieved through authorized systems to record vaccine dates and requirement status.

Parents do not self-certify the Blue Card. Provide the DVR, healthcare-provider record, prior-school record or other acceptable evidence and allow authorized staff to determine the student’s status.
School nurse script “Please tell me the exact vaccine, dose or document missing from the student’s file. Do you accept the California Digital Vaccine Record, and is the student complete, conditional, overdue or covered by an applicable medical exemption?”
Conditional admission and transfers

What happens when the school record is incomplete?

Conditional admission

A student who has started all required vaccine series but is not currently due for another dose may qualify for conditional admission under California’s schedule.

30-day review

Schools must review conditionally admitted students’ immunization records at least every 30 days until requirements are completed or an applicable exemption is filed.

Overdue later

Current implementation guidance says a school discovering an admitted student is overdue must notify the parent and allow no more than 10 school days for required vaccination or an applicable medical exemption before exclusion.

Homeless and foster transfer students

Immediate enrollment protection: California says qualifying homeless and foster students transferring into a new school may be enrolled immediately even when immunization records are unavailable, with up to 30 school days allowed while records are located.
This does not eliminate immunization requirements. The school still works to obtain the student’s records and bring the file into compliance.
Medical exemption route

A lost record and a California medical exemption are not the same thing

California uses CAIR-ME for new school and child-care medical exemptions. Parents/guardians begin the process in CAIR-ME and a California-licensed physician issues and manages the exemption when medically appropriate.

Missing record

Recover documentation from DVR, provider, pharmacy, school, Healthy Futures or another state.

Medical exemption

Use CAIR-ME only when a qualifying medical reason is being documented through the current physician process.

Do not use CAIR-ME simply because you cannot find a vaccine record. Record recovery should be attempted first unless an actual medical-exemption situation exists.
Pharmacy and healthcare-system recovery

Find vaccines that are present in a pharmacy or provider account but missing from DVR

Open the account used for the vaccination Check the pharmacy app, healthcare portal or patient portal connected with the appointment.
Search the immunization area Look under Immunizations, Preventive Care, Health Summary, Visit History, Documents or Pharmacy History.
Request the clinical administration record Ask for the vaccine name and date—not only a payment receipt.
Verify identity information Make sure the source record has the correct legal name and date of birth.
Ask which California registry received the report Determine whether the organization submitted to CAIR or Healthy Futures/RIDE.
Keep the original source document It can support a later registry correction or school/employer review.
Pharmacy script “I received the [vaccine] at your California location around [date], but it does not appear on my Digital Vaccine Record. Please provide the administration record and confirm whether the dose was successfully submitted to CAIR or Healthy Futures/RIDE.”

School archives may be limited. CDPH’s parent guidance warns that school immunization records often may not remain accessible for long after graduation, so do not rely on a former school as your only lifetime backup.

If the old provider closed

Call the former practice number The recording may identify the successor office or medical-record custodian.
Check affiliated hospitals or health systems Records may have transferred when a practice merged or closed.
Check former schools and colleges They may still have a previously accepted immunization record.
Check pharmacy history Adult vaccinations can remain easier to locate through the administering pharmacy.
Give verified historical records to your California provider Authorized providers can submit historical vaccine information to CAIR when appropriate.
Federal and special systems

Military, VA, Indian Health Service and other federal records

Vaccinations administered through federal systems may not be available through California DVR in the same way as ordinary California provider records.

Department of Defense

Use the applicable military health or personnel-record system.

Veterans Affairs

Use VA healthcare record services for vaccinations administered through the VA.

Indian Health Service

Contact the federal facility or agency that administered the vaccination.

CDPH guidance: People vaccinated through a federal agency may need to contact that agency directly for their immunization history.
Vaccinated outside California

Recover another state’s record before asking California to add historical doses

Contact the state where the vaccine was administered Use that state’s official IIS or the administering provider/pharmacy.
Save the original official record Keep the PDF, printout or healthcare-provider documentation.
Give the verified record to your California provider CDPH says California healthcare providers can submit historical doses to CAIR.
Request a new DVR afterward A previously saved copy will not refresh automatically.
No national database automatically combines every state. Search the jurisdictions where the vaccinations actually occurred.
CAIR privacy controls

What happens when you lock or unlock a CAIR record?

CAIR is an opt-out sharing registry. California requires reporting of immunization information, while patients and parents have a separate right to limit sharing of the CAIR record with other participating organizations.

Lock

Limits access by other CAIR organizations while allowing the organization responsible for the record—and applicable public-health access—to continue handling it under CAIR rules.

Unlock

Restores sharing of the record with other authorized CAIR users.

Locking does not delete the record. It changes sharing status. It is not the workflow for correcting a name, vaccine date or missing dose.
Ask your provider first: CDPH notes that healthcare providers may already be able to transmit your sharing preference electronically.

Frequently asked questions

Immunization records in California FAQs

How do I get immunization records in California online?

Use the official California Digital Vaccine Record portal. Select the California Immunization Record option, enter the requested identity and contact information, create a four-digit PIN, submit the request and open the secure record link if the registry finds a match.

Is the California Digital Vaccine Record an official immunization record?

Yes. CDPH describes the DVR as an official State of California immunization record that can be printed and used for purposes such as school, child-care entry and work.

Does California DVR show vaccines other than COVID-19?

Yes. The California Immunization Record option can show routine childhood and adult vaccines reported to the California registry, while the separate COVID-19 record provides COVID-specific information and QR proof.

How long do I have to open my California DVR?

You currently have 24 hours after receiving the secure link to enter the four-digit PIN and access the record. If the link expires or the PIN is forgotten, submit a new DVR request.

What if California cannot find my Digital Vaccine Record?

Try another legitimate phone number or email associated with the vaccination history, verify identity information with the provider, check Healthy Futures/RIDE when applicable, and use My DVR Assistant if the record still cannot be matched.

How do I correct a missing vaccine in CAIR?

Contact the healthcare provider or pharmacy that administered the vaccine. Ask it to verify the clinical administration and submit or correct the dose in the appropriate California immunization registry. The CAIR Help Desk cannot directly add missing doses for residents.

Why are older vaccines missing from my California record?

California’s broad requirement for providers to report administered immunizations took effect January 1, 2023. Older vaccinations may never have been submitted, and CAIR itself only began receiving records in the late 1990s and early 2000s.

Can I request records for several children?

CDPH’s current general FAQ says parents or legal guardians can request California Immunization Records for up to three minor dependents in one submission when their records are linked to the same cellphone number or email. The live request page also contains separate-request wording, so follow the workflow displayed by the portal. Multi-dependent submission does not apply to COVID-19 QR records.

What if my vaccinations were given in a Healthy Futures/RIDE county?

Healthy Futures/RIDE covers Alpine, Amador, Calaveras, Mariposa, Merced, San Joaquin, Stanislaus and Tuolumne counties. If a vaccination from those counties is missing, contact the administering provider and Healthy Futures Help Desk.

What if my vaccinations were given outside California?

Request the official record from the state registry, provider or pharmacy where the vaccination occurred. Give verified documentation to your California healthcare provider if you want the provider to review whether historical doses can be submitted to CAIR.
