Use PIERS when the vaccination history was reported to Pennsylvania’s statewide registry outside Philadelphia and you can complete Keystone Login plus the one-time-code verification.
- Adults age 18 or older can access their own record.
- Parents/legal guardians can access an eligible dependent under 18.
- Available records can be viewed, saved and downloaded.
Use the PIERS Update Request when the portal cannot find the record or demographic information needs updating.
Use the Authorization for Release of Immunization Records when you cannot receive the portal verification code, need a formal DOH release, or fall into another release-form situation.
Vaccines administered inside Philadelphia are reported to the city’s PhilaVax Immunization Information System rather than the statewide PIERS reporting route.
A person whose vaccines were split between Philadelphia and another Pennsylvania county may need both sources.

How to get a child’s Pennsylvania vaccine record
A parent or legal guardian can use the PIERS Public Portal for an eligible dependent when the child’s identity and parent-contact information match what the provider reported.
The normal parent/dependent portal route no longer applies. The adult should access their own record; a legal guardian needing access for an adult uses the appropriate PA DOH authorization route.
