How to download the South Carolina school vaccination certificate
Beginning August 1, 2025, South Carolina DPH made DPH Form 4024 available through the SIMON Public Portal for qualifying school-aged children in 5K through 12th grade.
Daycare families still need the provider route for Form 4024
The online school-certificate feature currently applies to qualifying children in 5K through 12th grade.
South Carolina DPH says parents and guardians still need to obtain DPH Form 4024 from the child’s healthcare provider for daycare because daycare immunization requirements differ from the school portal workflow.
Why SIMON may not find your vaccination record

Allow up to 10 business days for a new dose to appear
South Carolina DPH says vaccination records may take up to 10 business days to appear after the vaccination is administered.
Keep the provider or pharmacy administration record while the registry reporting process is still underway.
Continue checking SIMON, but do not immediately assume the provider failed to report the vaccination.
Ask the provider to confirm the patient demographics and whether the vaccination was successfully reported.
How to correct a South Carolina vaccination record
Start with the healthcare provider or pharmacy that actually administered the vaccine. That organization can verify the original clinical event and correct demographic or vaccination information before the record is updated in SIMON.

South Carolina DPH says a school principal or authorized representative may issue a Special Exemption to qualifying transfer students while medical records are being obtained, or to other qualifying students unable to secure immunizations or documentation immediately.
A missing record is different from a vaccination exemption
You believe the vaccination was already administered but cannot currently prove it.
Continue with SIMON, providers, pharmacies, old schools and previous state registries.
South Carolina has separate medical and religious exemption processes for applicable childcare and school requirements.
An exemption should not be treated as a shortcut simply because documentation is temporarily missing.

DPH says the portal uses patient information such as first name, last name, gender and date of birth together with contact information already associated with the record. A matching email address is important because the verification code is sent by email.
What is the difference between DPH Form 4024 and DPH Form 4025?
Form 4024 is the South Carolina Certificate of Immunization used for applicable school and childcare requirements. Form 4025 is the personal immunization history and does not replace the required school certificate.
Can I download my child’s Form 4024 through SIMON?
Parents and legal guardians can download DPH Form 4024 through SIMON for qualifying children in 5K through 12th grade when the child meets the applicable certificate requirements.
Can I get a daycare Form 4024 from the SIMON Public Portal?
Current DPH guidance says the online Form 4024 feature is for qualifying 5K–12 students. Parents and guardians still need to obtain the appropriate Form 4024 from the child’s healthcare provider for daycare.
Why can’t SIMON find my vaccination record?
Common causes include a different legal name, previous surname, mismatched email or phone information, a recent vaccine that has not appeared yet, vaccinations given before 2017, out-of-state records, military vaccinations or provider reporting problems.
How long can a recent vaccine take to appear in SIMON?
South Carolina DPH says vaccination records may take up to 10 business days to appear after administration. If a South Carolina vaccination remains missing afterward, contact the provider that administered it.
Will military or out-of-state vaccinations appear in SIMON?
Not necessarily. South Carolina DPH says vaccinations administered in another state or at a military facility may not be available in SIMON. Contact the original provider, military clinic or immunization registry where the vaccination occurred.
