How to submit the Authorization to Release Immunization Records
Use Washington’s official release form when MyIR, healthcare providers, pharmacies and schools cannot provide the complete record you need.
Processing-time difference:
Washington’s consumer webpage currently says a signed request is sent within five business days. The current official release PDF states no later than 15 business days and usually within 3–5 business days. If you have a hard deadline, plan around the longer 15-business-day window.

Telephone privacy:
Washington says staff can tell you whether a matching IIS record exists after obtaining identifying information, but vaccination details are not released over the phone.
Email-delivery privacy warning:
The official release form warns that information is not encrypted after it leaves WAIIS when sent through ordinary email. Consider whether fax or postal mail better fits your privacy needs.
Authorization expiration:
The current form states that unless terminated earlier, the authorization expires 18 years after it is signed or when the child turns 18, whichever occurs first.
Revocation:
The form allows a person to revoke the authorization through a written request to Washington IIS. Revocation does not undo disclosures already made before the request was received.

Do not describe every CIS as automatically medically verified. A validated WAIIS-generated CIS and a manually completed CIS have different verification workflows.

It is not simply “30 days after the first day of school.”
To qualify for conditional status, the child first needs all required doses they are currently eligible to receive.
