Which Utah record source should you use first?

1
Docket

Best first online route for personal or family records, PDF reports and recent-record checks.

2
MyUtah

Second official online route when you already use a Utah account or Docket does not help.

3
Provider or pharmacy

Best source for a recent vaccination, missing dose or original administration details.

4
Local health department

Useful for a USIIS printout, school help, public-clinic history or non-app access.

5
USIIS support

Use for unresolved record research, demographic problems, duplicates or access support.

USIIS is not guaranteed to contain every dose. Utah says participation is voluntary and not every provider submits records. Out-of-state, older, military, international, pharmacy or paper-only doses may require separate proof.

Complete access walkthrough

How to get Utah immunization records step by step

Ask what document the receiving office requires A school may need the USIR/Pink Card, while an employer, college, mission, camp or travel clinic may accept a Docket PDF or provider record.
Try Docket using exact USIIS identity details Use the first name, last name, date of birth, legal sex, phone number and email most likely stored in the state registry.
Try MyUtah as the second online route Sign in through the official Utah portal and locate the immunization-record service.
Ask the administering provider or pharmacy Request an Official USIIS Personal Utah Immunization Record or a complete vaccine-administration history.
Contact a local health department Ask whether staff can print the available USIIS history and what identification or guardian documentation is required.
Submit the official USIIS support request Use this for unresolved access, record research, demographic updates or possible duplicate records. Complete every field and attach photo ID securely.
Search every state or separate record system Check previous state registries, military records, foreign records, schools, employers and health systems when vaccinations occurred outside Utah.
Review the result before sending it Confirm the record owner, birth date, vaccine names, administration dates and whether the receiving organization accepts that format.
Save three secure copies Keep a private PDF, one printed copy and a protected backup. Avoid publicly sharing screenshots or QR codes.
Urgent deadline strategy: Run Docket and MyUtah, then contact the provider, pharmacy, school and local health department on the same day. Do not wait for one route to fail before starting the next.
Official Docket workflow

How to search, download and share a Docket record

Prepare the details used for registry matching

First name stored in USIIS
Last name or former surname
Exact date of birth
Legal sex stored in the registry
Current or previous phone number
Current or previous email address
Child details for a family-record search
Access to the verification phone or email
Open Docket from Utah’s official record page Mobile users can download the free app. Web users can choose the web application and select Utah.
Create or sign in to your account Utah’s Docket tip sheet shows email, Apple and Google login options. Use a private device and a secure account.
Choose adult or child records Enter the record owner’s information, not merely the account holder’s information.
Enter the registry details exactly The first name, last name, date of birth and legal sex must match the state record. A valid phone number or email must also be stored.
Complete verification Check the phone, email, spam and junk folders. Never give a verification code to an unsolicited caller or message.
Review the immunization history Compare the result with provider, pharmacy and old school records. Docket can only display information available and matched in the state registry.
Download the correct report Use the standard share icon next to the record owner’s name, choose the appropriate report and download the PDF.
Add another family member when needed Use the plus icon in the Immunization Search History area to start another search.
Official Docket PDF details: Utah’s tip sheet explains that users can access back-to-school PDF records, add family members, view vaccine status and drag down to refresh after a new vaccination.
Docket troubleshooting

Fix no-match, verification and missing-dose problems

Docket problem and the best next action
Problem Likely reason What to do next
No record found Name, birth date, legal sex or contact details do not match USIIS. Try former names and old contact details, then ask the provider or USIIS support to verify demographics.
Verification code never arrives The registry has an old, missing or inaccessible phone number or email. Check old accounts, then use the provider or official support route to update the registry contact.
Recent vaccine is missing The provider has not reported it, it failed to match, or Docket has not searched again. Ask the administering provider to verify its USIIS submission, then drag and release to refresh.
Only part of the history appears Vaccines may be split across duplicate profiles or other states. Ask support to check for duplicates and contact every state where vaccines were administered.
Child record does not appear The child’s identity or parent contact does not match USIIS. Ask the pediatrician to verify the child’s demographics and parent or guardian information.
App says records were found but nothing loads The search or display has not refreshed correctly. Drag down on the records screen. If unresolved, remove the search and perform a new one.
Use drag-and-release for a true registry refresh. Utah’s Docket guidance says a recent vaccination will appear only when it was correctly reported to the state and the records screen searches again.
A missing app result is not proof that a vaccine was never given. It may remain in a provider, pharmacy, school, military, foreign or another state’s record system.
Second online route

Use MyUtah when Docket does not solve the request

Utah DHHS lists MyUtah as another way to access available personal or family immunization histories, review reports and share records.

Sign in through Utah.gov

Use the official MyUtah address and your existing state account where available.

Locate the immunization service

Search the available services for immunization or vaccination records after signing in.

Separate login from record problems

A password issue belongs to MyUtah support. A missing vaccine or wrong demographic field belongs to the provider or USIIS route.

Parent and guardian access

How to get a child’s Utah immunization record

Utah DHHS says a person requesting the record of a child under 18 must be the child’s parent or legal guardian at the time of the request.

Search Docket or MyUtah using the child’s details Enter the child’s exact name, date of birth and legal sex plus the parent phone number or email most likely stored in USIIS.
Ask the pediatrician for an official record Request an Official USIIS Personal Utah Immunization Record and confirmation that all administered doses were reported.
Ask the school what it already holds The school may have a verified USIR/Pink Card or a transferred immunization history from a prior school.
Contact the local health department Ask what parent identification or guardianship documentation is required before visiting.
Use the secure support request when needed Complete every required field and attach a readable copy of photo identification through the official form.
Pediatrician call script “I need my child’s complete Utah immunization history. Please confirm the child’s exact name, date of birth, legal-sex field, my parent or guardian contact details and whether every vaccine was submitted to USIIS. Can you print an Official USIIS Personal Utah Immunization Record?”
Secure state support

Prepare a complete USIIS support request

Utah’s official parent page directs residents to a secure support request for their own or a child’s record and says the form must be completed fully with a copy of photo identification.

Gather this information before opening the form

Full current legal name
Former, maiden or alternate names
Exact date of birth
Current phone number and email
Previous phone number or email
Child’s details for a guardian request
Provider or pharmacy names
Approximate vaccination dates
Description of the access problem
Clear copy of valid photo ID
Open the form from Utah’s official page Confirm the destination uses the Utah health department’s secure RedCap system.
Describe the exact result you need Explain whether the issue is a record request, name correction, outdated contact, duplicate profile or missing online access.
List providers and approximate dates This helps staff distinguish a missing match from a vaccination that was never reported.
Upload identification through the secure form Do not send photo ID to an unofficial website or social-media account.
Save the submission confirmation Keep the submission date, confirmation number and a private copy of any files sent.
Missing clinical information

How to correct a missing or incorrect USIIS dose

Utah’s USIIS page directs people who need to add a missing immunization back to the healthcare provider. The administering source must verify the clinical information before the official record is corrected.

Information to collect before requesting a correction
Detail Where to find it Why it matters
Vaccine name Provider portal, pharmacy history, receipt or paper card. Identifies the exact product that should appear.
Administration date Appointment history, claim, receipt or clinical record. Helps the provider locate the original encounter.
Administering location Clinic, pharmacy, school event or public health record. Shows which organization is responsible for verification.
Manufacturer or lot number Provider or pharmacy administration record. Supports clinical verification when available.
Identity used at the visit Insurance profile, portal account or appointment confirmation. May reveal a duplicate record or demographic mismatch.
Provider correction script “My Utah immunization record is missing or incorrectly showing the [vaccine] given at your location around [date]. Please verify the administration record, patient name and birth date, and whether the dose was submitted to USIIS. If it was rejected or attached to another profile, can your office correct the source record?”
After a correction: Allow time for the provider submission to process, then drag and release on the Docket Immunization Records screen to search again.
Utah school record

USIR and Utah Pink Card requirements explained

The Utah School Immunization Record, commonly called the Pink Card, is the official immunization certificate used by Utah schools. It must be supported by verified vaccine information rather than guessed dates or an unverified list.

What may be used to complete the Utah school record
Documentation What it must show School action
USIIS-printed USIR Available Utah registry vaccinations and student details. Review accuracy, completeness and required supporting documents.
Provider or pharmacy record Vaccine name and month, day and year of administration. Transfer verified information to the USIR.
Previous-school record Prior verified immunization history. Use the transferred information and retain the current USIR.
Proof of immunity Qualifying laboratory results and a healthcare-provider statement for applicable diseases. Attach the supporting document to the USIR.
Exemption form The applicable personal, religious or medical exemption. Attach the official exemption documentation; medical exemptions also require provider support.
Student name and birth date are correct
Every dose includes month, day and year
Provider or pharmacy source is identifiable
Out-of-state records were included
Immunity evidence is attached when used
Exemption documentation is attached when applicable
School personnel verified the completed record
A parent copy was saved before submission
A blank form is not accepted proof by itself. The USIR must contain accurate, verified vaccination information and any required exemption or immunity documents.
School-office script “Do you need a USIR printed from USIIS, or can you use my Docket PDF or provider record to complete it? Please identify the exact missing vaccine, dose date or supporting document.”
Enrollment deadlines

What happens when a school record is incomplete?

Utah school review and conditional-enrollment timeline
Stage Official rule or timeline Family action
Enrollment submission School requests an immunization record, exemption or qualifying immunity proof. Submit the clearest complete document and keep a copy.
School review The school reviews compliance and identifies deficiencies within five school days. Ask for the missing vaccine or document in writing.
Conditional enrollment The student may generally attend while the record is under review or for 30 calendar days after notice. Use Docket, provider, local health and previous-school routes immediately.
Written notice Notice identifies the deficiency and warns that attendance may end without compliance. Respond with the exact missing item rather than resending the same incomplete screenshot.
Possible extension An administrator may allow extra time for medical necessity or certain extenuating circumstances. Ask the principal or school nurse what written evidence is required.
Noncompliance The student may be excluded until acceptable documentation is provided. Contact the school and healthcare source immediately to complete the missing requirement.
No school review fee: Utah’s school-rule training says schools cannot charge a fee for receiving or reviewing immunization records or exemption forms.

Special situations

Student experiencing homelessness

The student must be enrolled immediately even when immunization documentation is missing. Ask for the district’s McKinney-Vento liaison.

Military family

Children of military families receive 30 days from enrollment to meet Utah immunization requirements.

Transfer student

The previous school must provide the immunization record when requested by the legally responsible person or when student records are transferred.

Adults, college, work and pharmacy

Choose the right record for the receiving organization

Adult record need and the best first source
Reason for proof Check first Ask before submitting
College or university Docket, MyUtah, provider, old school and campus health portal. Does the college accept a PDF, or must its own form be completed?
Healthcare employment Occupational health, provider, pharmacy and former employer. Which dose dates, laboratory results or signed forms are required?
Mission or volunteer work Docket, MyUtah, provider and the organization’s medical paperwork. Is a full history required or only specific vaccinations?
Travel Travel clinic, pharmacy, provider and international vaccination card. Does the clinic require original documentation or a specific certificate?
Immigration examination USIIS/provider record, foreign documents and civil-surgeon instructions. Which records or laboratory evidence the civil surgeon will accept.
Personal adult history Docket, MyUtah, primary-care portal and pharmacy history. Whether older or out-of-state doses are absent from the available registry record.

Pharmacy record recovery

Use the pharmacy profile used for booking
Try the phone number used at the appointment
Search old appointment emails
Call the exact store location
Ask for a complete vaccine-administration history
Confirm whether each dose was reported to USIIS
Correct the pharmacy demographic profile
Refresh Docket after the correction
Do not repeat a vaccination or pay for laboratory tests solely because an app is incomplete. Ask a qualified healthcare professional and the receiving organization what documentation or testing is appropriate.
Old or incomplete history

How to rebuild a lost Utah immunization record

Search personal and family files Check baby books, paper cards, school forms, camp forms, mission paperwork, military records and old medical folders.
Contact former healthcare providers Ask whether the practice still holds records or transferred them to a successor office, hospital system or records custodian.
Contact previous schools and colleges Ask specifically for immunization documentation rather than only an academic transcript.
Check pharmacies and former employers Pharmacy profiles and occupational-health departments may retain vaccine-administration records.
Search all previous states Contact the registry and administering provider in each state where a dose was given.
Bring recovered records to a Utah provider Ask the provider to evaluate the documents and add verified historical information where appropriate.
No single national immunization registry exists. Records may remain divided among state registries, providers, pharmacies, schools, military systems and personal files.
Participation status

USIIS withdrawal and re-enrollment forms

Utah provides separate official forms to withdraw a person from USIIS or re-enroll later. These forms should not be used merely to fix a Docket login or ordinary demographic error.

Withdrawal

The record will no longer be included in or shared through USIIS with authorized healthcare providers, health insurers, schools, day care centers or publicly funded programs.

Re-enrollment

The record may again be included in USIIS and shared with authorized participating organizations.

Information requested by the forms

Self or child request selection
Last, first and middle name
Date of birth
Mother’s maiden name
Previous name when withdrawing
Parent or guardian details for a child
Street address
Phone number
Email, fax or mail confirmation choice
Signature and date
Published processing estimate: The October 2025 withdrawal and re-enrollment forms say to allow two to four business days for processing.
Cross-state record recovery

Vaccines given outside Utah require another search

USIIS and Docket may not automatically display vaccinations administered elsewhere. Contact the registry, provider or pharmacy in the state where each dose was given.

Common questions

Utah immunization records FAQs

How do I get Utah immunization records online?

Start with Docket or MyUtah, which Utah DHHS lists as public routes for available personal and family immunization records. If online matching fails, ask a healthcare provider or local health department for an Official USIIS Personal Utah Immunization Record or submit the official USIIS support request.

Can the public log directly into USIIS?

USIIS portal access is mainly for authorized providers, schools, early childhood programs and public health users. Residents generally use Docket, MyUtah, a provider, a local health department or the official patient support request.

Why can Docket not find my Utah vaccine record?

The first name, last name, date of birth, legal sex, phone number or email may not match USIIS. The record may also be duplicated, withdrawn, unreported, stored by a provider or pharmacy, or held in another state.

How do I refresh a recent vaccination in Docket?

Open the Immunization Records screen and drag down, then release, to search the state registry for updated information. A recent dose appears only after the administering provider correctly reports and matches it to USIIS.

Can I request my child’s Utah immunization record?

Yes. Utah DHHS says a request for a child under age 18 must be made by the child’s parent or legal guardian. Use the child’s exact registry details and be prepared to provide photo identification through the secure support route.

What is the Utah Pink Card?

The Utah Pink Card is the common name for the Utah School Immunization Record, or USIR. It is the official school immunization certificate and must contain verified vaccine information, exemptions or qualifying immunity documentation when applicable.

How long can Utah conditional school enrollment last?

A student may generally remain on conditional enrollment for up to 30 calendar days after the school provides notice of missing or incomplete immunization documentation. Certain extensions or special protections may apply.

How do I correct a missing dose in USIIS?

Contact the healthcare provider, pharmacy or clinic that administered the dose. Ask it to verify the vaccine, date, patient demographics and USIIS submission. Utah directs people seeking to add missing clinical records back to the provider.

Does USIIS contain every vaccination I have received?

No. Provider participation is voluntary, and records can be incomplete when doses were not submitted, were given outside Utah, predate electronic reporting, are stored in military or federal systems, or cannot be matched to the correct profile.

Who should I contact for Utah immunization record help?

The live Utah DHHS USIIS pages and CDC list 801-538-9450 for Utah immunization system assistance. The official patient support form is usually the best route for record research, demographic corrections or unresolved access problems.
