How to request a California Digital Vaccine Record

California’s public DVR portal retrieves available vaccinations reported to CAIR. It can provide a broader California Immunization Record or a COVID-19-only record.

This county process applies when the vaccinations were administered through an Orange County Health Care Agency clinic. It does not automatically retrieve doses from Kaiser, Hoag, UCI Health, Providence, CVS, Walgreens, Costco or another private source.

The current English form contains six numbered parts. It states that all numbered items must be completed for the authorization to be valid and that its formatting must not be changed.

What happens when two CAIR profiles exist?

CDPH provides an online duplicate-record correction route for healthcare providers. The provider should verify that the profiles are truly duplicates before requesting a merge because the merge process cannot be reversed.

Patients should not submit a provider-only merge request themselves. Ask the provider or pharmacy to investigate the duplicate and work with the CAIR Help Desk.
CAIR privacy choices

Can a California record be locked or unlocked?

CDPH states that patients and parents may review their own or their child’s CAIR record for accuracy. They may also request that sharing with other authorized CAIR users be limited through the lock process and may later request unlocking.

Locking is not deletion

The record remains in CAIR. Locking changes how it is shared with other authorized users.

Speak with the provider first

CDPH advises checking whether the provider already sends sharing preferences electronically before filing a separate form.

Do not use a lock request to correct an error. Missing or incorrect vaccinations require provider verification or a registry correction route.
School and childcare proof

How Orange County schools use vaccine records

A parent provides source documentation such as DVR, a provider record, a prior-school record or an immunization-registry record. School or childcare staff use that source to complete and update California’s official school immunization record, commonly called the Blue Card.
