CA Digital Vaccine Record 2026: Step-by-Step Retrieval Guide

California · DVR · CAIR · Healthy Futures/RIDE · record recovery

Retrieve, Review and Fix Your California Vaccine Record

California’s Digital Vaccine Record is the main public route for accessing an available immunization history reported to the state’s immunization systems. The correct workflow is different from the provider-facing CAIR2 login, and regional Healthy Futures/RIDE records can matter for vaccinations given in eight California counties.

Use this guide to request your own or a child’s record, match old contact information, open the secure link, recover missing doses, use My DVR Assistant, check regional or out-of-state sources, prepare school or work proof and protect your CAIR privacy settings.

Privacy warning: This page cannot retrieve a California vaccine record. Do not enter your birth date, child’s information, telephone number, email, 4-digit PIN, ID.me credentials, QR code, photo ID or medical record on an advertisement or unofficial “instant vaccine record” website. Enter private information only on verified California government, healthcare-provider or pharmacy systems.

Choose the California record task you need

Start with the task matching your problem instead of troubleshooting every possible record source.

First decision

Use the correct California record route before troubleshooting

Most residents Digital Vaccine Record

Use California’s public DVR when you need your own available record or an authorized child’s record.

  • Full California Immunization Record.
  • COVID-19 QR record.
  • School or childcare proof.
  • Work or personal medical files.
Missing clinical dose Provider or pharmacy

Use the organization that administered the vaccination when the DVR is missing a dose or shows an incorrect administration entry.

  • Verify original administration date.
  • Check patient demographics.
  • Ask whether the dose was reported.
  • Request a CAIR correction when appropriate.
Eight-county region Healthy Futures / RIDE

Vaccination providers in eight California counties report through the Healthy Futures regional registry.

  • Alpine.
  • Amador.
  • Calaveras.
  • Mariposa.
  • Merced.
  • San Joaquin.
  • Stanislaus.
  • Tuolumne.
Another jurisdiction Other state or federal agency

Vaccines administered outside California or by certain federal agencies may require a separate record request.

  • Another state’s IIS.
  • Veterans Affairs.
  • Department of Defense.
  • Indian Health Service.
  • Other federal systems.
Fast deadline strategy: If school, work or a clinical program needs proof urgently, request the DVR and contact the provider or pharmacy at the same time. An immediate provider printout may solve the deadline while the registry issue is reviewed.
Choose the correct output

Full California Immunization Record vs COVID-19 QR record

Choose the record type matching the receiving organization’s request
Record What it contains Best for Important limitation
California Immunization Record Vaccinations reported to the California registry, vaccination dates and available future vaccine recommendations. School, childcare, work, healthcare records and personal history. Not every historical vaccination is guaranteed to have been reported.
COVID-19 Vaccine Record COVID-19 dose information and a SMART Health Card QR code when available. COVID-specific proof when the recipient accepts that format. It is not a complete routine immunization history.
Provider record Vaccinations documented in that healthcare organization’s medical system. Missing CAIR doses, urgent proof or older history. It may not include vaccines administered elsewhere.
Pharmacy history Vaccines administered by that pharmacy or pharmacy network. Recent adult vaccines and missing pharmacy doses. It is not necessarily the complete statewide history.
Understand the output

What can appear on a California Digital Vaccine Record?

Patient name
Date of birth
Vaccination dates
Vaccines received
Dose count by vaccine group
Future vaccine recommendations when available
Complete status
Overdue status
Future due date
Invalid-dose label when schedule logic flags a dose
DVR status is not the same as a school, employer or medical decision. California’s DVR applies immunization-schedule logic to the registry history. A school, employer or other program may apply separate requirements, and a healthcare professional should interpret clinical questions.
Official online workflow

How to request a CA Digital Vaccine Record

Open the official California DVR request page Verify that you are on myvaccinerecord.cdph.ca.gov before entering private information.
Choose the record type Select the full California Immunization Record or the COVID-19 Vaccine Record with QR code.
Enter the patient’s first name Use the spelling most likely stored in the registry.
Enter the patient’s last name If the first request later fails, investigate maiden names, former surnames and spelling differences.
Enter the date of birth The current form uses MM/DD/YYYY.
Provide a mobile phone number or email Start with current contact information. Matching depends on information associated with the vaccination record.
Create a private four-digit PIN Save the PIN temporarily because it is required to unlock the secure record link.
Complete the authorization Confirm that you are the patient or authorized parent/guardian and complete the current privacy acknowledgements.
Submit and watch the matching phone/email If a matching record is located, the secure access link is delivered to the contact method associated with the record.
Open the link, enter the PIN and save the record Review the entire record and store a private PDF or printed copy.
Prevent a failed search

Prepare the identity details most likely to match CAIR

Current legal first name
Current legal last name
Former surname
Maiden surname
Hyphenated-name version
Exact date of birth
Current mobile number
Old mobile numbers
Current email
Old emails used with providers
Phone number used by a pharmacy account
Parent contact used for a child’s vaccines
Do not randomly guess identity combinations. Use details that were actually associated with your healthcare, pharmacy or vaccination history. If the DVR still fails, confirm the demographic data with the provider.
Important after saving

Your downloaded DVR does not automatically update

A saved California Digital Vaccine Record is a copy of the information available when you requested it. New vaccinations do not automatically appear inside a PDF, image or record you already saved.

After another vaccination: allow the provider’s reporting process to occur, then return to the California DVR portal and request a fresh copy.
Parents and guardians

How to get a child’s California vaccine record

Use the child’s identity information Enter the child’s first name, last name and date of birth—not the parent’s vaccination details.
Use the matching parent/guardian contact information The email or mobile number may be the contact given to the pediatrician, pharmacy or vaccination clinic.
Create a private PIN Do not use a publicly shared or easily guessed PIN.
Complete the parent/guardian declaration Follow the authorization shown by the official request form.
Review every expected vaccine Compare the DVR with the pediatrician’s chart when a school-required dose appears to be missing.
Save a private backup before enrollment deadlines Keep both a secure digital copy and, when useful, a printed copy.
Pediatrician script “I am trying to retrieve my child’s California Digital Vaccine Record. Please confirm the legal name, date of birth and contact information stored in the record, and verify whether all vaccines your office administered were submitted to the California immunization registry.”
Family records

Requesting records for more than one child

California’s current DVR FAQ says parents or legal guardians can request California Immunization Records for up to three minor dependents in one submission when the records are linked to the same mobile number or email address.

Full California Immunization Record

Multi-dependent functionality may be available for up to three minor children when the matching contact information is shared.

COVID-19 QR record

California says the multi-submission feature does not apply to COVID-19 Vaccine Records with QR codes.

Follow the live request screen. Portal workflows can change. If the current request form instructs you to submit children separately, follow the instruction displayed during your actual request.
Common Google-search mistake

CAIR2 login is not the normal resident vaccine-record account

The California Immunization Registry application login is an authorized-user system for approved healthcare, school, childcare, public-health and other permitted organizations.

Resident / patient / parent
  • Use the Digital Vaccine Record portal.
  • Use My DVR Assistant for unresolved record issues.
  • Contact the provider for missing clinical doses.
  • Contact CAIR or Healthy Futures for appropriate support.
Authorized CAIR2 user
  • Uses an organization code.
  • Uses an approved username/password.
  • Access is connected with authorized organizational work.
  • The login is not a consumer registration shortcut.
If Google takes you to a page warning “DO NOT ATTEMPT TO LOG ON UNLESS YOU ARE AN AUTHORIZED USER,” stop. That is not the normal California resident DVR request route.
California regional registry route

Healthy Futures/RIDE matters in eight California counties

California providers located in the following counties use the Healthy Futures/Regional Immunization Data Exchange route for immunization-registry reporting:

Alpine County
Amador County
Calaveras County
Mariposa County
Merced County
San Joaquin County
Stanislaus County
Tuolumne County
When this route matters: If an expected vaccine was administered by a provider in one of these counties and the DVR remains incomplete, ask the provider whether the dose was entered into Healthy Futures/RIDE and contact the regional registry for record help when appropriate.
Healthy Futures record script “I need an immunization record for vaccines administered in [county]. The California Digital Vaccine Record is missing information. Can you tell me whether the record may be in Healthy Futures/RIDE and what public record-request process I should use?”
No record found

Why the California DVR may fail to find a match

Current phone does not match The registry may contain an older mobile number used at vaccination.
Email does not match A former provider, pharmacy or work email may still be attached.
Former surname The record may have been created under a maiden or earlier legal name.
Name spelling differs Hyphens, suffixes, spacing or provider spelling can affect matching.
Date of birth error An incorrect DOB in the source record can block a clean match.
Duplicate patient profile Separate records can sometimes exist for the same person.
Provider did not report the dose The clinical chart may contain vaccination information absent from the DVR.
Older pre-2023 history Historical routine doses may be less complete.
Out-of-state vaccination Another state IIS may hold the record.
Federal vaccination VA, DoD, IHS or another federal source may need to be searched separately.

Use this troubleshooting order

Retry with the other contact method If current mobile fails, try the email connected with vaccination history.
Check historical contact information Review old phone numbers and emails actually used with providers.
Check former names Compare the exact patient name in the provider or pharmacy record.
Ask the provider to confirm the CAIR demographic record Verify legal name, DOB and contact information.
Use My DVR Assistant Escalate the unresolved match through California’s official review system.
Check Healthy Futures, another state or federal source when relevant Do not assume every vaccination must be located in one CAIR search.
Official escalation route

When to use My DVR Assistant

My DVR Assistant is California’s official route for certain unresolved Digital Vaccine Record problems.

Good reasons to use it
  • DVR still cannot locate a record.
  • Personal information is incorrect.
  • Contact information is incorrect.
  • An underlying record review is needed.
  • Provider corrections have not resolved access.
Eligibility and verification
  • You need to be a California resident.
  • You need at least one vaccine dose administered in California for this DVR-assistance route.
  • Identity verification may use ID.me.
  • Follow the secure official workflow only.
Minor rule: California’s current Assistant says someone younger than 18 generally needs a parent or legal guardian to submit the request, unless the person is an emancipated minor age 16 or older.
Follow-up timing: California says users are generally notified of review findings and follow-up actions within about a week. Treat this as guidance rather than a guaranteed deadline.
Record correction

How to fix a missing or incorrect vaccine dose

When a dose itself is missing or clinically incorrect, start with the healthcare provider or pharmacy that administered it. The source organization is normally best positioned to verify the original vaccination event and submit the missing or corrected information.

Build a correction packet before contacting the provider
Detail Where to find it Why it helps
Vaccine name Provider portal, vaccine card or pharmacy history. Identifies the missing vaccination event.
Administration date Clinical note, appointment history or pharmacy record. Helps staff find the exact encounter.
Administering location Clinic, pharmacy, hospital or public-health site. Identifies the organization responsible for verification.
Manufacturer / lot Clinical administration record when available. Adds evidence for the event being reviewed.
Patient identity used Provider or pharmacy profile. Can reveal a duplicate or mismatched record.
Provider correction script “My California Digital Vaccine Record is missing or incorrectly showing the [vaccine] administered at your organization around [date]. Please locate the original administration record, confirm the patient information used and verify whether the dose was correctly submitted to CAIR or Healthy Futures/RIDE.”
The CAIR Help Desk is not the clinical source for adding a missing dose. California directs patients with missing or incorrect vaccination doses back to the healthcare provider for the underlying update.
Why historical completeness varies

California’s vaccine-reporting rules changed on January 1, 2023

California AB 1797 requires immunizing healthcare providers to report administered immunizations to a California immunization registry.

Vaccinations from 2023 onward

All vaccine doses administered in California are subject to the current statewide registry-reporting requirement.

Vaccinations before 2023

Older routine immunization histories may be incomplete because the reporting requirements were not the same.

Important pre-2023 distinction: California’s DVR FAQ says that before January 1, 2023, COVID-19 vaccines and vaccinations administered at pharmacies were required to be reported, while the same statewide requirement did not apply to every routine provider-administered vaccine.
Reporting geography: Providers in most California counties report to CAIR. Providers in Alpine, Amador, Calaveras, Mariposa, Merced, San Joaquin, Stanislaus and Tuolumne use Healthy Futures/RIDE.
New dose not showing

What to do when a recent vaccination is missing

Check the provider or pharmacy record first Confirm that the vaccination was documented in the source clinical system.
Ask whether it was submitted to the registry Verify CAIR or Healthy Futures/RIDE reporting.
Allow normal reporting processing A recently administered dose may not appear immediately.
Request a brand-new DVR Your old saved record will not update automatically.
Escalate only if the fresh DVR remains incomplete Return to the provider for a missing dose or use My DVR Assistant for an unresolved record-access issue.
Moved into California

How to add documented out-of-state vaccines to your California history

Retrieve the original out-of-state record Use the previous healthcare provider, pharmacy or state IIS.
Give the documentation to a California healthcare provider Ask the provider to review the outside record.
Ask whether the documented doses can be submitted to CAIR California’s DVR guidance allows a California provider to submit documented out-of-state doses.
Request a fresh DVR after the update Do not rely on a record downloaded before the change.
Federal record route

Military, VA and other federal vaccinations may need a separate request

California’s DVR guidance says vaccinations administered by certain federal agencies may require direct contact with that agency because the information may not be available through the normal California record.

Department of Defense

Use the applicable military medical-record or health-record route.

Veterans Affairs

Retrieve vaccination information from the VA health-record system or medical-record office.

Indian Health Service

Contact the IHS or tribal healthcare facility that administered the vaccination.

A missing CAIR entry does not prove a vaccine was never administered. Federal, military, out-of-state and historical systems can hold separate pieces of a person’s vaccination history.
Using the record

California DVR for school, childcare and work

California describes the DVR as an official state immunization record that can be used for school/childcare entry and work.

Match the record to the receiving organization’s actual request
Situation Useful record Confirm before submitting
Childcare Full California Immunization Record or provider documentation. Age-specific requirements and whether the facility needs another form.
K–12 school Complete dated immunization history. The exact missing vaccine or dose identified by school staff.
College DVR plus provider or student-health documentation as required. Campus-specific vaccine and upload requirements.
Healthcare work DVR, provider records and other evidence accepted by occupational health. Which exact vaccines or other medical evidence the employer requires.
General employment Official DVR when vaccine proof is actually required. Whether the employer needs the entire record or only one vaccination.
Do not use the DVR’s “Complete,” “Overdue,” “Due” or “Invalid” labels as a substitute for the receiving organization’s requirements. Ask the school, employer or program which exact documentation it needs.
DVR status explanation

What does “Invalid” mean on a California DVR?

California’s DVR may label a dose “Invalid” when the dose does not fit the immunization timing or schedule logic used by the registry.

That does not automatically mean the dose fails every school or employer requirement. California specifically advises users to consult their healthcare provider because receiving organizations may apply different rules.
Do not repeat a vaccine solely because the DVR says “Invalid.” Vaccination and revaccination decisions belong with a qualified healthcare professional.

Spanish-language access

California DVR website translation vs record language

California’s DVR website provides automatic page translation through Google Translate.

Website interface

Users can translate navigation and instructions into other languages.

Saved/printed record

California says the California Immunization Record PDF is currently available only in English.

Do not alter the official PDF to create your own translated medical record. If an organization needs translation, ask what translation format it accepts and keep the original record intact.
CAIR privacy controls

California patients can request to lock or unlock CAIR sharing

California requires immunization information to be reported to its registry systems, but patients and parents have privacy rights concerning how records are shared with other CAIR users.

Lock Limit sharing with other CAIR users

A patient or parent can use California’s lock process when they want to limit registry sharing with non-clinical CAIR sites under the state’s rules.

Unlock Restore sharing status

If the patient or parent later changes their mind, California also provides an unlock process.

Locking is not deletion and is not a correction workflow. Do not use the lock/unlock form to fix a missing vaccination or incorrect vaccine date.
Spanish forms are available. California’s CAIR Forms page currently provides lock and unlock forms in English, Spanish and several other languages.

Frequently asked questions

CA Digital Vaccine Record FAQs

How do I get my CA Digital Vaccine Record?

Use California’s official Digital Vaccine Record request page. Choose a full California Immunization Record or COVID-19 Vaccine Record, enter the patient’s first and last name, date of birth and matching mobile number or email, create a four-digit PIN and submit the authorized request.

How long do I have to open the California DVR link?

California currently gives you 24 hours from delivery of the secure link to access the record with the four-digit PIN you created. If the link expires or you forget the PIN, start a new DVR request.

Why can’t California find my Digital Vaccine Record?

Common causes include an old phone number, old email, former surname, spelling difference, incorrect date of birth, duplicate profile, provider reporting problem, older pre-2023 vaccination history, out-of-state vaccination or a vaccination held by a federal system.

Can I request California vaccine records for multiple children?

California’s current FAQ says a parent or legal guardian can request California Immunization Records for up to three minor dependents in a single submission when their records are linked to the same mobile number or email. The multi-submission option does not apply to COVID-19 QR records. Follow the live request form if its workflow differs.

How do I fix a missing dose on my California DVR?

Contact the healthcare provider or pharmacy that administered the vaccine. Ask it to verify the original vaccination event and submit or correct the dose in CAIR or the applicable California registry. The CAIR Help Desk does not independently add a missing clinical dose for a patient.

What is Healthy Futures/RIDE and do I need it?

Healthy Futures/RIDE is the regional immunization registry used by providers in Alpine, Amador, Calaveras, Mariposa, Merced, San Joaquin, Stanislaus and Tuolumne counties. It can be an important record source when vaccinations were administered in those counties.

Are all California vaccinations required to be reported?

Since January 1, 2023, California requires immunizing providers to report vaccine doses they administer to a California immunization registry. Older routine vaccination histories may be less complete because earlier reporting requirements differed.

Can I use a California Digital Vaccine Record for school or work?

California describes the DVR as an official record that can be used for school or childcare entry and work. The school, childcare program, employer or other receiving organization still determines the specific vaccination and document requirements that apply.

Does my saved California DVR update automatically after another vaccine?

No. A record you already saved does not automatically update. After another vaccination is reported, return to the DVR portal and request a fresh record.

What should I do if my vaccines were given outside California or by the military or VA?

Retrieve out-of-state vaccinations from the provider or immunization registry where they were administered. A California provider may be able to submit documented out-of-state doses to CAIR. Vaccinations administered by federal agencies such as the Department of Defense, Veterans Affairs or Indian Health Service may require a separate request directly from that agency.
