A Colorado CIIS record can be official and still be incomplete
CIIS combines immunization information reported to the Colorado registry by healthcare providers, pharmacies and some schools. The Public Portal itself warns that printed records may not contain every vaccination.
CIIS is not your complete medical chart
It is an immunization registry. Diagnoses, medications, visit notes and most other clinical information remain in provider medical records.
Reporting matters
A vaccination can exist in a doctor or pharmacy record without appearing in your current CIIS output if it was not correctly reported or matched.
Outside records may remain separate
Another state’s IIS, military care, federal healthcare or international records may still need a separate search.

Open Colorado’s official CIIS Public Portal
Use the official public portal rather than a healthcare-provider CIIS login or a third-party vaccine-record lookup website.
Select “Me” or “Dependent”
Choose “Me” for your record. Choose “Dependent” only when you are authorized to request that dependent’s record.
Enter the exact first name
Use the name documented by the healthcare provider rather than a nickname unless the provider actually recorded the nickname.
Enter the exact last name
Consider a maiden name, previous surname, hyphenated surname or other legitimate variation used at vaccination visits if your current name does not match.
Enter date of birth and gender
Both must match the existing provider-submitted CIIS information.
Choose mobile phone or email verification
The contact information used for verification must match what the healthcare provider has on file.
Request the access code
Check SMS, inbox, spam and junk folders.

Colorado provides a separate access-support form for people who are having difficulty accessing their own or their child’s record through myColorado or the CIIS Public Portal.
Verify the identity information once more

Submit only one request
The official form warns that submitting multiple forms increases response delay.
Watch for secure-email follow-up
The CIIS Help Desk says it follows up using a secure email message sent to the address supplied in the form.
Do not submit duplicate support forms.
One complete request is better than repeatedly submitting the same problem.
When to use CDPHE’s Request to Release Immunization Record Form
This is different from the access-help form. It is the formal manual route for individuals and parents or guardians of minors who need CIIS records released outside the self-service portal process.

Common rejection reasons:
CDPHE says incomplete forms, requests without proper ID, or requests without required supporting documentation will not be accepted.
How approved records are delivered:
CDPHE currently says immunization records requested through this route are securely transmitted to the requester by fax or secure email.
Do not rely on an old copied form.
Open the live CDPHE record-copy page immediately before submitting because supporting-document and form instructions can change.
Choose “Dependent” and enter the child’s exact provider-documented information plus the requester contact information used for verification.

Colorado school vaccination records and the new seventh-grade Tdap timing
Before the first day of school, Colorado requires families to provide each school the student attends with an acceptable immunization document, a healthcare-provider-signed in-process plan, an exemption certificate, or a combination covering different requirements.

What if my vaccinations were given outside Colorado?
