New York State Immunization Records: NYSIIS & CIR 2026

New York statewide recovery guide · NYSIIS, CIR and usable proof

Find the Source Holding Your Vaccine History

New York records can be divided between NYSIIS, New York City’s CIR, providers, pharmacies, schools and other states. The right first step depends on where each vaccination was administered and reported.

Use this guide to retrieve a printable record, find a child’s history, correct missing doses, complete an NYC paper request, meet a school deadline or rebuild an older adult record.

Protect your medical information: This independent guide cannot access NYSIIS or CIR. Do not submit a portal password, Social Security number, photo ID, birth certificate, access code or medical details here. Enter private information only after opening an official government, provider, pharmacy or school system.

New York State route Vaccination given outside New York City

NYSIIS is the statewide registry used by providers outside Manhattan, Brooklyn, Queens, the Bronx and Staten Island.

  • Ask the administering provider to print the record.
  • Ask the county health department to check NYSIIS.
  • Check pharmacy, school and college health records.
  • Use NYSDOH assistance if the usual sources fail.
New York City route Vaccination given in one of the five boroughs

NYC uses the Citywide Immunization Registry, or CIR. Eligible individuals, parents and guardians can begin with My Vaccine Record.

  • Search available records online.
  • Download an official report.
  • Correct identity or missing-dose problems.
  • Use online assistance, mail or fax when access fails.
Routing example: A Queens resident vaccinated in Westchester may need the NYSIIS route. A Nassau County resident vaccinated in Manhattan may need CIR. Use where the dose was administered, not only the current address.

How residents can obtain a NYSIIS-related record

NYSIIS is not designed as a universal consumer download account. The most useful route is usually an authorized organization that already knows the patient.

Contact the provider that gave the vaccination Ask for the complete immunization history and whether the office can print the NYSIIS record.
Check the provider portal Look under Immunizations, Preventive Care, Health Summary, Medical Records or Documents.
Contact the local health department Ask whether staff can search NYSIIS and which identity or authorization documents are required.
Check the pharmacy Adult flu, COVID-19, RSV, shingles, pneumonia and Tdap records may remain in the pharmacy system.
Contact the school or college health office It may retain a provider-verified record previously submitted for attendance.
Ask staff to check former identity details Mention a maiden name, former surname, previous address or other legitimate information used during earlier care.
Use NYSDOH record help Prepare the legal name, date of birth, previous names, county, provider and approximate vaccination dates.
Parent route for children under 19: New York’s NYSIIS provider FAQ directs parents and guardians to request a child’s record from the primary care provider, local health department or NYSDOH Bureau of Immunization.
Provider or county call script “I need a complete immunization history for [school, work or personal use]. The vaccinations may have been given by [provider] in [county] around [year]. Can you check the clinical record and NYSIIS, including any profile under my former name, and tell me which identification you need?”
Avoid the wrong login

Why most residents should not start at the NYSIIS login

Resident, patient or parent

Begin with:

  • Primary care provider.
  • Vaccinating pharmacy or clinic.
  • Local health department.
  • School or college health office.
  • NYSDOH record assistance.
Provider or authorized organization

NYSIIS access normally involves:

  • An individual Health Commerce System account.
  • An authorized organization role.
  • Required training.
  • Privacy and patient-access responsibilities.
Do not share HCS credentials. The NYSIIS login contains confidential patient information and is intended for approved users. A resident who reaches the login screen should return to the provider or local-health-department route.

How to use My Vaccine Record correctly

My Vaccine Record searches immunizations reported to NYC Health. A successful match depends on the identity, relationship and contact details held in CIR.

Prepare these details before searching

Full legal name used by the provider
Date of birth
Current or former surname
Current and previous mobile numbers
Current and previous email addresses
IDNYC number when available
NYS DMV driver or non-driver ID when available
Parent or guardian details for a child
Open the official My Vaccine Record service Confirm that the page is the New York City service before entering personal details.
Select your own record or a child’s record Adults age 18 and older can access their own available record.
Enter the matching identity information Use the spelling, surname, phone or email most likely supplied to the provider.
Review every vaccine and date Compare the result with provider, pharmacy and paper records.
Download the complete official report NYC311 says My Vaccine Record printouts can be used for school, college and camp enrollment.
Use the assistance route when needed Do not edit a screenshot or build an unofficial replacement record.
Record coverage: NYC providers must report vaccines given to children age 18 and under. Adult records may be incomplete because only some adult vaccinations are reported.
Parent and guardian access

How to retrieve a child’s New York vaccine record

Child vaccinated outside NYC

Ask the pediatrician, clinic or local health department to retrieve the NYSIIS record.

Child vaccinated in NYC

Use My Vaccine Record when the requesting adult is correctly connected to the child in CIR.

Child vaccinated in several places

Compare registry, pediatrician, pharmacy, school and previous-state records.

NYC relationship matching

NYC311 says the adult must be listed on the child’s birth certificate or immunization record as a parent, guardian, next of kin or primary contact.

Pediatrician verification script “I cannot retrieve my child’s registry record. Please confirm the child’s exact name and date of birth, my parent or guardian relationship, the phone number or email attached to the profile, and whether every administered vaccination was successfully reported.”
Prevent the next mismatch: Ask the provider to confirm the parent or guardian contact details every time the child receives a vaccine.
Troubleshooting

No record found or only part of the history appears?

Former name mismatch Check a maiden name, previous surname, suffix, hyphenated name or provider spelling.
Old contact details Try a previous phone, parent phone, family email, work email or school email.
Wrong registry Check CIR for NYC-administered doses and NYSIIS-related sources for vaccinations given elsewhere in New York.
Provider did not report the dose Ask the administering office or pharmacy to verify its registry submission.
Duplicate patient profiles Ask the provider or registry-support route to investigate whether the history is divided.
Out-of-state vaccination Contact the registry and provider in the state where the dose was administered.
Older paper-only record Check pediatricians, former schools, family files, employers and military records.
Closed provider Find the successor practice, hospital network or medical-record custodian.

NYC escalation order

Review the My Vaccine Record help information Recheck the identity, contact and relationship details.
Ask the provider to verify CIR reporting Request confirmation that the dose and patient profile were submitted.
Submit one online Record Assistance Request Upload valid photo identification through the official form.
Allow the stated response period NYC311 says staff usually answer the online request within two to three days.
Use mail or fax if online access remains unavailable Send the official application with a copy of valid photo ID.
Email safety: NYC Health advises users not to send personal identifying information through ordinary email.
Missing or incorrect dose

How to request a registry correction

Start with the provider, pharmacy or clinic that administered the vaccination. The administering organization is usually the best source for clinical verification.

Information that helps locate and correct a dose
Detail Where to find it Why it matters
Vaccine name Provider portal, pharmacy history, card or receipt. Identifies the clinical entry that should appear.
Administration date Visit history, insurance claim or pharmacy receipt. Helps staff find the original encounter.
Administering location Clinic, pharmacy, school event or employer clinic. Shows which organization must verify the dose.
Manufacturer and lot Clinical or pharmacy administration record. Provides stronger clinical verification when available.
Identity used at the visit Appointment confirmation or patient profile. May reveal a duplicate or mismatched registry record.
Provider correction script “My immunization record is missing the [vaccine] administered at your location around [date]. Please verify the clinical administration record, the patient identity information used and whether the dose was accepted by NYSIIS or CIR or attached to another profile.”
Do not invent an administration date. An estimated date can create school, employment and medical-record problems. Use verified documentation or ask the receiving organization which alternative evidence it accepts.

Which immunization documents can New York schools accept?

Acceptable record formats identified by NYSED
Record type What it provides Practical action
NYSIIS or CIR record An official New York registry history. Ask the authorized provider, health department or NYC portal for the complete record.
Signed practitioner certificate Vaccine products and administration dates. Request a full immunization certificate rather than an ordinary visit summary.
Electronic health record Provider-generated electronic vaccination history. Download the complete report and confirm the school accepts it.
Another state registry Official record from the state where a dose was given. Use the correct state registry and provide the full result.
Official foreign record Vaccination history issued in another country. Ask whether translation or healthcare-provider review is required.
Previous-school record A complete record forwarded by the former school. Ask the former school to send it directly rather than carrying an informal copy.
Qualifying immunity evidence Certain accepted laboratory results or verified disease history. Ask the school and healthcare provider before ordering tests.
Signature detail: NYSED says NYSIIS/CIR reports, electronic provider records, another state’s electronic registry record and an official foreign record do not require a healthcare practitioner’s signature. Other document types may require one.

In-process student documentation

At least the first required dose in each vaccine series
Age-appropriate appointments for remaining doses
Appointment card, receipt or provider statement
Written date of the next appointment
Follow-up within the required catch-up intervals
Updated documentation after each dose
Important timing: NYSED says appointment documentation must support timely catch-up. A student who fails to receive later doses within the required interval can lose in-process status and may be excluded.
Special enrollment situations

Record rules for students facing documentation barriers

Practical actions from New York school guidance
Situation School rule What to do
Student in temporary housing The student can be enrolled even without ordinary immunization documents. Ask for the district or charter-school McKinney-Vento liaison to help obtain records.
Student in foster care Enrollment should proceed even when ordinary records are unavailable. The enrolling school should request the record from the former school, which must forward it within five days of receiving the request.
Newly arrived refugee student Special time may be allowed for assessment and beginning required vaccinations. Bring every written foreign record to the health screening and school review.
Homeschooled student Home instruction is treated differently, but proof may be required for certain school-building activities. Ask before testing, sports or activities not open to the general public.
Graduating senior NYSED encourages schools to provide immunization certificates to graduates. Request a copy before graduation and store it with transcripts.
School record-retention backup: Student immunization records must be kept for at least six years, or three years after the student reaches age 18, whichever period is longer. Ask early because records may not remain available indefinitely.
Adults, pharmacies and employment

How to recover an incomplete adult vaccination history

NYSIIS can contain records for people of all ages, and CIR may contain adult vaccinations reported with consent. Older adult records can still be incomplete.

Best first source for common adult record needs
Situation Check first Request
Flu, COVID-19, RSV or shingles dose The exact pharmacy account and store location. A complete pharmacy immunization history.
Healthcare employment Employee health, provider, pharmacy and registry sources. The employer’s written proof requirements.
College or clinical training Student health, former school and provider. Accepted vaccine dates, forms and laboratory formats.
Military vaccination Military medical, TRICARE or VA systems. A complete service immunization history.
Travel-clinic vaccine Travel clinic and international vaccination card. Vaccine product, date, lot and clinic verification.
Do not repeat a vaccine only because it is absent online. Take all available documentation to a qualified healthcare professional and ask the receiving organization what evidence it accepts.
Old or closed practice

How to find records after a provider closes

Call the old practice number A recorded message may identify the successor practice or medical-record custodian.
Contact the affiliated hospital system The private practice may have transferred records to a larger healthcare network.
Ask the former insurer Claims may identify the provider, pharmacy and approximate vaccination date.
Contact previous schools and colleges Ask specifically for the immunization documentation submitted during enrollment.
Ask the local health department Staff may identify public clinics or help with the NYSIIS route.
Search the provider’s licensing record It may identify a relocation, retirement or successor office.
Preserve the original: Scan or photograph an old paper card before allowing a provider, school or employer to copy it.
Deadline rescue plan

What to do when proof is needed urgently

Needed today

Call the receiving office and provider. Ask for the exact missing dose and whether a provider fax, registry report or temporary document is accepted.

Needed within 2–3 days

Use My Vaccine Record for NYC, contact the provider or local health department outside NYC, and submit the online NYC assistance form when appropriate.

One week or longer

Collect provider, pharmacy, school and out-of-state records, then request an early document review.

Receiving-office script “Please tell me the exact vaccine or dose that is missing, the deadline and every format you accept. Can I use a NYSIIS or CIR report, provider electronic record, pharmacy record, previous-school transfer or another state’s registry?”
Do not wait on only one source. When a deadline is close, contact the registry route, provider and receiving organization at the same time.
Moving to or from New York

How to combine records from other states

List every state where vaccines were administered Include childhood, college, military, employment and pharmacy vaccination locations.
Contact each state registry separately There is no single public national portal combining all state records.
Request provider and pharmacy copies Registry information may not include every older adult dose.
Give verified records to the current provider Ask whether the outside history can be added to NYSIIS or CIR.
New NYC residents: NYC Health advises people moving from another location to give their new provider a copy of previous immunization history so it can be added to CIR.

Common questions

New York State immunization records FAQs

How do I get New York State immunization records?

First identify where the vaccine was administered. Outside New York City, ask a healthcare provider, pharmacy, school or local health department to retrieve or check the NYSIIS record. For New York City vaccinations, use My Vaccine Record or the Citywide Immunization Registry request route.

Can a resident log directly into NYSIIS?

The NYSIIS Health Commerce System login is primarily for authorized healthcare, school and public-health users. Residents normally obtain records through a provider, local health department, pharmacy, school or New York State record-assistance route.

Can I get an NYC immunization record online?

Eligible adults, parents and legal guardians can use My Vaccine Record to search available CIR records. Identity, relationship and contact details must match the information reported to the registry.

How can a parent get a child’s New York immunization record?

Outside NYC, request the child’s record from the primary care provider, local health department or New York State Bureau of Immunization. In NYC, use My Vaccine Record when the requesting adult is correctly listed as a parent, guardian, next of kin or primary contact.

What should I do if no immunization record is found?

Check former names, old phone numbers, providers, pharmacies, schools, local health departments and other state registries. Ask the administering provider to verify reporting and check for duplicate or mismatched patient profiles.

Which immunization records can New York schools accept?

New York school guidance recognizes NYSIIS and CIR records, certain signed provider certificates, electronic health records, qualifying laboratory evidence, another state’s official registry, official foreign records and complete records forwarded by a previous school.

Can an in-process student continue attending school?

A student who meets New York’s in-process requirements may continue attending while completing required vaccine series. The family must provide written proof of future appointments and follow the required timing.

Why might an adult New York vaccine record be incomplete?

Older vaccinations may predate electronic reporting, adult reporting may depend on consent, and some doses may remain only in pharmacy, provider, employer, military, college or out-of-state records.

How long does an NYC record request take?

NYC311 says online Record Assistance Requests are usually answered within two to three days and mail or fax requests take about one to two weeks. The official paper application states that a response should be received within 10 business days.

What if vaccinations were administered outside New York?

Contact the registry, provider, pharmacy or former school in every state where vaccinations were administered. There is no single public national portal that automatically combines all state records.
