Get the official South Carolina school certificate

South Carolina’s Certificate of Immunization is DPH Form 4024. Current DPH guidance says it is the record accepted for school and childcare attendance.

Search the child in SIMON Complete the parent/guardian verification process.

Look for “Download School Certificate” The option is available for qualifying school-aged children when SIMON can generate an acceptable Form 4024.

Download the complete Form 4024 Save the original file before printing.

Print it exactly as issued DPH says the portal-generated Form 4024 is accepted as printed from the portal.

Give it to the school without altering it Do not add handwritten vaccine information or electronically modify the certificate.

Online school certificate: eligible parents/legal guardians have been able to obtain the 5K–12 Form 4024 through SIMON since August 1, 2025.

No handwriting or electronic editing. If the certificate is wrong or incomplete, fix the underlying provider/SIMON information and generate a corrected certificate.

Daycare, preschool and 4K

Daycare families still need the provider certificate route

The current SIMON online Form 4024 feature is for qualifying 5K–12 children. DPH says parents and guardians still need to obtain the appropriate Form 4024 from a healthcare provider for children entering daycare because childcare requirements differ.

Why pre-2017 vaccinations may not appear in SIMON

South Carolina DPH states that, beginning January 1, 2017, immunization providers are required to report administered vaccine doses to the state immunization registry.

DPH currently says SIMON does not receive immunization information from the military. Military families should contact the clinic associated with the military installation.

SC school/childcare use

DPH Form 4375, Military Certification of South Carolina School and Childcare Immunization Requirements, can be used with the applicable military vaccination documentation.

Separate school processes

A missing record is not the same thing as an immunization exemption

Medical exemption

A qualifying medical exemption is completed through the licensed healthcare-provider process on the applicable South Carolina Certificate of Immunization.

Religious exemption

DPH currently provides the South Carolina Certificate of Religious Exemption through local health departments, including in-person Form 1126 and an online Form 1126A route.

Special Exemption

A school principal or authorized representative can issue a one-time temporary Special Exemption in qualifying record/documentation-delay situations.

Special Exemption timing: current DPH guidance says it is valid for only 30 days and can be issued only once.

Do not use exemption forms simply because a record is temporarily missing. Follow the school’s official process and use the 30-day transfer/document route only when the school determines it applies.
