Delaware’s final 2026 amendment to 14 DE Admin. Code 804 requires documentation specifying the month, day and year immunizations were administered by a licensed healthcare practitioner.

What Delaware’s conditional-admission rule means
Under the current public-school rule, conditional admission requires a statement from a licensed healthcare practitioner showing at least the minimum initial vaccinations specified in Regulation 804.
A destroyed provider record is not the same as a DelVAX no-match
Regulation 804 has a separate pathway when the student’s immunization record was actually lost or destroyed by the medical provider that administered the vaccine.
Medical and religious exemptions have their own Delaware processes
Delaware uses a School Vaccination Medical Exemption Form. When the situation does not meet the criteria on that form, a supplemental form may also require DPH review and approval.
