What does NYC CIR contain?

CIR is New York City’s confidential immunization information system. My Vaccine Record can only return vaccinations that have actually been reported to the City and matched to the person.

Children age 0–18

NYC healthcare providers must report vaccines they administer to children age 18 and younger.

Adults age 19+

Adult vaccinations can be reported with the patient’s oral or written consent, so an adult CIR history may be incomplete.

Older NYC history

NYC warns that people born in the city before 1995 may not have complete immunization information in CIR.

Age distinction that causes confusion: My Vaccine Record lets an individual age 18 or older request their own record, but the special adult CIR reporting-consent rule begins at age 19. Vaccines given to patients age 18 or younger are still under mandatory pediatric reporting.

CIR is not a complete medical chart. It is an immunization registry. Hospital notes, diagnoses, medications, laboratory results, imaging and unrelated clinical records must be requested from the healthcare organization that created them.
