How the system actually works

Ontario vaccine records can be held in several different places

Public Health Ontario tells people seeking an immunization record to contact their local public health unit. Canada.ca gives the same Ontario-specific direction.

That does not mean a PHU necessarily has every vaccine administered throughout your life.

Best recovery principle: trace each missing vaccine back to the organization or jurisdiction that actually administered or recorded it.

Find the correct public health unit before entering private information

Open Ontario’s current PHU directory Use the provincial directory because public-health boundaries and organization names can change.

Identify the PHU connected to the record For school records, this will commonly be the PHU serving the student’s school. Follow the PHU’s own jurisdiction instructions.

Open the PHU’s official immunization page Look for “immunization records,” “report vaccines,” “student immunization” or “ICON.”

Open ICON from that official PHU page Avoid submitting a health card number into an unrelated search-result page.

Save the PHU support number You may need it for OIID, PIN, identity matching or age-16 access.

How to use Immunization Connect Ontario (ICON)

ICON is Ontario’s online immunization-record tool used through local public health units.

Enter ICON from your PHU website This helps ensure you use the system connected to the correct public-health record.

Choose whether you are viewing or reporting a record Follow the options presented by your local ICON portal.

Verify the person The portal may use the Ontario Health Card number or an Ontario Immunization ID.

Create or enter the PIN Follow the local PHU’s PIN instructions.

Review the vaccinations currently on file Compare them with the yellow card, doctor and pharmacy records.

Open the printable record Where supported, choose the printable PDF/electronic yellow-card option.

Report missing doses Use the secure reporting process and upload supporting proof when requested.

Check the record again after processing A submitted update may need PHU review before it appears.

ICON only shows what is on the public-health record. An incomplete ICON history does not prove the person was never vaccinated.

No Ontario Health Card? Current Toronto and York Region procedures demonstrate that an OIID and PIN can be used in some local ICON workflows. Contact your own PHU for its exact procedure.

Do not repeatedly guess a PIN. Contact the PHU for its reset/recovery process.

Teen record privacy

Record access can change when a student turns 16

Ontario PHU procedures can place greater control of the record with the student once they reach age 16.

Toronto example: Toronto Public Health currently says a student’s ICON account automatically locks on the 16th birthday and requires a PIN reset. The student decides whether to keep sole access or consent to parent/legal-guardian access.

This is not a reason to create a second account. If parent access stops when the child turns 16, contact the PHU responsible for that record.

Use the electronic Yellow Card without losing the paper record

Ontario continues to use “yellow card” terminology for a personal immunization record.

Once the public-health record is available through ICON, the portal can provide an electronic yellow-card style record for viewing or printing.

Never type a missing vaccine into an electronic Yellow Card yourself. Update the underlying public-health record through the authorized process.

How to report a vaccine to the Ontario public-health record

Important current Ontario rule: do not assume the healthcare provider automatically updated the child’s public-health student record. Ontario’s current public guidance tells parents/guardians to report updated child immunizations to their PHU.

Find proof of the vaccination Use the provider record, pharmacy history, yellow card or another legitimate source document.

Open the correct PHU reporting page Use ICON or the alternate reporting method that PHU publishes.

Copy the information carefully Enter the vaccine/product and administration date exactly from the source.

Upload proof when requested Make sure the image is readable and contains enough information for verification.

Submit through one official route Do not flood the PHU with duplicate copies by email, fax and ICON unless staff asks you to.

Allow time for validation Updates are not necessarily visible instantly.

Check ICON again Confirm the dose and date display correctly after processing.

Processing times vary by PHU. For example, York Region currently says ICON updates can take up to five business days to appear. Do not treat that local estimate as a guaranteed Ontario-wide timeline.

Request the complete history Ask for more than the latest appointment summary.

Compare it with ICON Identify dates present in the provider record but absent from public health.

Recover pharmacy-administered vaccines separately

Open the pharmacy account used for the vaccine Check old email addresses or family profiles when necessary.

Adult vaccine recovery often requires several record holders

Canada.ca’s current Ontario record guidance directs residents to their healthcare provider or local health unit. For an adult lifetime history, provider and pharmacy records can be especially important.

Childhood vaccines Check former schools, old providers and the paper yellow card.

Adult booster Check the doctor or clinic that administered it.

Pharmacy vaccine Recover the pharmacy’s own administration record.

Travel vaccine Contact the travel clinic.

Workplace vaccine Check occupational-health records.

Another province Request the vaccine history from that province/territory.

School immunization records are reviewed by the local public health unit

Under Ontario’s Immunization of School Pupils Act (ISPA), the medical officer of health maintains and reviews immunization records for pupils attending school in the PHU’s area.

Toronto’s current guidance: parents/legal guardians or students age 16+ must report vaccinations to Toronto Public Health. Toronto explicitly says healthcare providers do not report those student vaccines to TPH under the current process.

York Region gives the same practical instruction: parents/guardians must provide the child’s immunization information to York Region Public Health.

What to do if public health says the student’s record is incomplete

Read the notice line by line Determine whether public health says a dose, record or valid exemption is missing.

Compare it with the Yellow Card The vaccination may already have been received.

Check the provider record Obtain source documentation for the disputed or missing entry.

Report existing proof immediately Use the PHU method identified in the notice.

Check that the PHU processed the update Do not assume uploading the image instantly closed the case.

Contact the PHU when the notice still shows an issue Ask which exact entry remains outstanding.

A missing record and a missing vaccination are different problems. Do not make a medical decision based only on an incomplete online record.

Child-care immunization records follow a different legal framework

Ontario Regulation 137/15 requires licensed child-care operators to address immunization for children who are not already attending school, subject to the regulation’s exemption provisions.

Ask the licensed child-care centre what record it needs Do not assume its intake process is identical to the child’s future school process.

Provide the vaccination information requested Use the Yellow Card, provider documentation or another accepted source.

2026 record-access change

Ontario COVID-19 vaccine-record access changed on March 13, 2026

March 13, 2026: Ontario’s enhanced COVID-19 vaccine certificates stopped being available for printing or download through the former provincial process.

Ontario currently says COVID-19 vaccine records will become available again for viewing or downloading through local PHU ICON portals, or by contacting the PHU, starting in Fall 2026.

Current timing: this guide is being updated in August 2026. Fall 2026 access is therefore still an announced upcoming rollout. Confirm current availability with Ontario and your local PHU before depending on it.

Contact the pharmacy Request the administration record if the pharmacy gave the dose.

How do I get an Ontario COVID-19 vaccine record in 2026?

Ontario stopped making enhanced COVID-19 vaccine certificates available for printing or download on March 13, 2026. Ontario says COVID-19 vaccine records are planned to become available through local PHU ICON portals or PHU contact starting in Fall 2026. Until the rollout is available locally, check existing saved records, the administering pharmacy/provider and your PHU.

What if my vaccines were given outside Ontario?

Request the original record from the provider, province, territory, U.S. state, country, school or public-health authority where the vaccination occurred. Keep the original and ask your Ontario PHU or healthcare provider how the historical doses should be documented when necessary.
