Submit, Clear or Retrieve Your UF Immunization Record
University of Florida immunization records involve two very different tasks. Incoming students usually need to satisfy UF Health Compliance and clear an immunization hold, while current or former students may instead need a copy of immunization records already maintained by the UF Student Health Care Center.
Use the route below for Florida SHOTS records, out-of-state vaccines, manual UF form uploads, program requirements, international TB documentation, waivers, rejected submissions, Preview holds, old SHCC records or missing historical vaccinations.

Health Compliance and SHCC Medical Records solve different problems
Use UF Health Compliance, the Student Self-Service Portal and ONE.UF. This is the route for registration holds, required vaccines, forms, screening and waivers.
Use the SHCC Medical & Immunization Records Request process. Do not re-upload a compliance form simply to obtain a copy of an existing chart.
Use the former-student/non-student release route and check the 10-year SHCC retention rule.
Search the original provider, state registry, pharmacy, former school or college, military records and other locations where vaccinations were given.
Which University of Florida students need immunization documentation?
- New University of Florida students.
- Students taking classes or participating in course activities at UF locations.
- Non-degree-seeking students.
- International students, who also have TB-screening requirements.
- Most health-related majors, which can have additional requirements.
Certain distance learners may qualify for a group exemption.
How to submit University of Florida immunization records
Check for Florida SHOTS records before manually uploading
UF says Florida SHOTS information may pre-populate an admitted student’s immunization account about one week after UF acceptance when matching Florida records are available.
How to request another state’s immunization record through UF
What types of immunization records can UF consider?
UF currently lists several possible documentation routes. Health Compliance makes the final determination about whether an alternative record can be used instead of the UF Mandatory Immunization Form.
Have the appropriate healthcare office complete and verify the form required for your academic program.
Useful when the vaccine history is available from Florida SHOTS or another state’s official registry.
Useful for transfer or graduate students whose previous institution already maintained verified immunization documentation.
UF lists military vaccination records as another possible record source.
UF General Education immunization requirements
| Item | Current UF route | Documentation point |
|---|---|---|
| MMR | Two MMR doses or qualifying positive laboratory evidence of immunity. | The form says titers must include the laboratory report. |
| Hepatitis B | At least one dose, qualifying immunity evidence or signed UF waiver. | Use UF’s current waiver process when declining. |
| MenACWY | One qualifying dose on or after age 16 or signed UF waiver. | UF’s troubleshooting page also tells students to check that the dose was given in 2005 or later. |
| TB screening | Required for international General Education students. | Testing must fall within the 12 months before starting UF classes. |
Before the healthcare provider signs the UF form
Program requirements can be significantly different
| Program | Current published requirements | What to verify separately |
|---|---|---|
| Health Professions | Two MMR doses or qualifying titer; completed Hepatitis B series or qualifying titer; MenACWY on/after age 16 or waiver; Tdap within the last 10 years; two Varicella doses or qualifying titer; required TB testing. | Use the current Health Professions form for exact TB requirements and check for additional clinical-rotation or residency requirements after matriculation. |
| Veterinary Medicine | MMR requirement/immunity route; Hepatitis B requirement/immunity/waiver route; MenACWY requirement/waiver route; Td/Tdap within the last 10 years; rabies clearance; TB screening for international students. | Use the current Veterinary Medicine form for detailed rabies clearance instructions and any current timing rules. |
TB screening documentation has its own failure points
UF’s General Education form says international students must complete TB screening within the 12 months before the start of UF classes. Health Professions students should use their program-specific TB instructions.
- Date the test was placed.
- Date the test was read.
- Result measured in millimeters.
- Negative or positive interpretation.
- Testing date.
- Result.
- Copy of the laboratory report.
- UF General Education form specifies the report should be in English.
Use UF’s own waiver or exemption process
| Route | Current UF process | Common mistake |
|---|---|---|
| Hepatitis B waiver | Read the applicable CDC Vaccine Information Statement, check the decline option on the UF form and provide the required legal signature and date. | Typing the student’s name instead of providing the required legal signature. |
| MenACWY waiver | Review the applicable CDC information, select the UF decline option and sign/date the form. | Submitting a waiver created for another university. |
| Medical exemption | UF says temporary or permanent exemption can be considered after receipt of provider documentation; contraindication documentation should be on provider letterhead. | Submitting only a student statement without the documentation UF requires. |
| Personal tenets / religious exemption | UF says the student must provide the K-12 immunization exemption record and then sign UF’s exemption-transfer form. | Assuming a prior exemption automatically transfers without the UF process. |
Minor students have extra signature and consent steps
UF’s current General Education form tells students under 18 to include a signed Minor Medical Treatment Consent Form.
Why UF records get rejected or stay incomplete
What to do when the immunization hold is still showing
What if Preview or registration is approaching?
UF’s current General Education form tells students to submit missing documentation at least three weeks before orientation or registration.
Current UF Preview guidance says the immunization hold can be temporarily lifted for Preview registration while submitted records are still processing.
Do not assume Preview’s temporary process will solve a missing submission. Complete the Health Compliance workflow as soon as possible.
UF says required vaccines are available at SHCC
If the problem is a genuinely missing required vaccination rather than missing documentation, UF says required vaccines are available through the Student Health Care Center and that SHCC accepts most health-insurance plans.
Use the secure UF submission workflow when possible
UF’s current form directs students to enter vaccine dates and upload the form through the Student Self-Service Portal.
Use the Student Self-Service Portal for immunization date entry and required document upload.
The General Education form warns that ordinary internet email is not necessarily secure and confidentiality cannot be guaranteed.
How to request immunization records already held by UF SHCC
This workflow is for records maintained by the UF Student Health Care Center. It is different from the incoming-student Health Compliance submission process.
Use the SHCC Medical Release / ROI form designated for current students.
Use the SHCC records-release form designated for former students or non-students.
How long does UF Student Health keep medical records?
SHCC currently says UF student medical records are retained for 10 years following enrollment inactivity or graduation. If the student re-enrolls within that period, the record is reactivated.
Current published SHCC copy-charge structure
| Record portion | Current published charge |
|---|---|
| First 10 pages | No charge. |
| Next 25 pages | $1 per page. |
| Pages after the first 35 | $0.25 per page. |
| Record-copy labor | $7 per hour when applicable. |
| Postage | Based on mailing weight. |
| Fax | SHCC currently says up to two pages can be faxed without charge. |
Can a parent obtain an adult UF student’s medical records?
SHCC says it generally may not release information about the student’s medical care to a parent, guardian or other third party without the patient’s express permission.
SHCC states that medical records for patients 17 and younger are available to parents/legal guardians under the applicable minor-patient rules.
Where to search when UF does not have the vaccine record
| Source | What to request | Useful when |
|---|---|---|
| Primary-care doctor / pediatrician | Complete official immunization history. | Childhood and routine vaccinations are missing. |
| State immunization registry | Official registry history. | Vaccines were reported in the state where they were administered. |
| Pharmacy | Vaccine-administration history. | Flu, COVID-19, Tdap or other pharmacy vaccines are missing. |
| Previous college or university | The official immunization documentation previously accepted. | You are a transfer, graduate or returning student. |
| Military / federal records | Official service vaccination history. | Vaccinations occurred through military or federal healthcare. |
| Former high school | Health-office immunization record. | You recently finished high school and childhood records are missing. |

University of Florida immunization records FAQs
How do I submit immunization records to the University of Florida?
Check your ONE.UF immunization To-Do Items, open the UF Student Self-Service Portal, review what is already on file, enter documented vaccine or screening dates and upload the required UF form or supporting records in the same session. Then check ONE.UF after review.
Will Florida SHOTS records automatically appear at UF?
UF says Florida SHOTS information may pre-populate an admitted student’s immunization account about one week after acceptance when matching Florida records are available. Check View Missing Immunizations and ONE.UF rather than assuming the hold is cleared.
How long does UF take to review an immunization submission?
UF currently tells students to allow about one week for verification after manual submission. This is a planning estimate rather than a guaranteed processing time.
How early should I submit UF immunization records?
UF’s current General Education form tells students to submit missing documentation at least three weeks before orientation or registration.
Why is my UF immunization hold still showing?
Possible causes include missing portal date entry, missing supporting documents, an unsigned or unstamped form, vaccine timing problems, missing titer reports, incomplete TB documentation, use of the wrong waiver or use of the wrong program form. Check ONE.UF and View Missing Immunizations.
Does UF email me when my immunization form is received?
UF’s current General Education form says Health Compliance does not send confirmation that an individual form has been received. Students should check their UF account to determine whether the immunization checklist has cleared.
Can an out-of-state student request vaccine records through UF?
UF currently provides an out-of-state registry-request workflow in the Student Self-Service Portal. Students select the state or states where vaccinations were given, enter associated identity/address information and check the Immunizations page later for updates.
What happens if my immunization records are still processing at Preview?
Current UF Preview guidance says the immunization hold can be temporarily lifted for Preview registration when records have already been submitted and are still processing. Required documentation still must be submitted and approved before classes begin.
How do former students request UF immunization records?
Former students should use the former-student/non-student SHCC records-release process when the record is held by Student Health. SHCC currently says student medical records are retained for 10 years after graduation or enrollment inactivity.
Can my parent request my UF medical records if I am over 18?
SHCC generally cannot release an adult student’s medical information to a parent, guardian or other third party without the patient’s express permission. The adult student can request their own records or provide an appropriate authorization.
