When the $5 copy fee does not apply: The City currently states that an immunization record is provided at no charge whenever an immunization is administered. The $5 charge is for a record-only request.
Call before traveling. Metro Health warns that the clinic can close early when capacity is full. Confirm the current record-copy process, required identification and whether you need to schedule anything before visiting.

Metro Health’s current Patient Authorization / Restriction for Release of Medical Records form is used to authorize or restrict release of protected health information to specified people or organizations.

Expiration: The current Metro Health form states that an immediate-release authorization expires after the specified disclosure is completed. A future-release authorization generally expires after three years or when a minor turns 18, subject to any earlier specified expiration.
Do not assume notarization is always mandatory. The current form includes a “For Notary Use Only (If Applicable)” area. Call Metro Health when you are unsure whether notarization or another proof requirement applies to your situation.

Provisional enrollment

Texas allows provisional enrollment in specific circumstances when the student has received at least one dose of each required age-appropriate vaccine and is not overdue for the next dose. The school reviews provisional status every 30 days.

A lost or incomplete record is not itself a vaccine exemption. The current Texas 2026–27 guidance specifically says parents cannot claim an exemption merely because fixing a lost or incomplete record is inconvenient.

No. F11-13366 grants adult registry participation consent. F11-11406 authorizes release of an available official immunization history.
