Request and Download the Right South Carolina Immunization Record
If you are searching for “SC DHEC immunization records,” you are using the former agency name. South Carolina public-health services are now handled by the Department of Public Health, while vaccination histories are accessed through SIMON, the Statewide Immunization Online Network.
This guide shows how to retrieve your own or a legal dependent’s record, choose DPH Form 4025 versus Form 4024, fix a failed SIMON match, recover older or military vaccinations and prepare the correct document for school, childcare, college or work.
Use the official South Carolina DPH SIMON Public Portal for an available statewide vaccination history. The public portal can search for your own record or a legal dependent’s record. After identity verification, choose Download Full Record for DPH Form 4025 or, when eligible, Download School Certificate for DPH Form 4024.

DPH Form 4024 vs DPH Form 4025
| Record | Purpose | Important limit |
|---|---|---|
| DPH Form 4024 | South Carolina Certificate of Immunization used for school and childcare attendance documentation. | The online SIMON certificate is currently limited to qualifying 5K–12 children; daycare families may still need a provider-issued certificate. |
| DPH Form 4025 | South Carolina Personal Immunization Record showing available vaccination history. | It is not the school/childcare Certificate of Immunization. |
| Provider record | Useful for correcting SIMON, proving a recent dose or supplying history to another healthcare provider. | A school can still require the official South Carolina certificate. |
| Pharmacy administration record | Useful for pharmacy-administered vaccines missing from the registry. | It may contain only doses given by that pharmacy chain/location. |
How to get the South Carolina school immunization certificate
Starting August 1, 2025, parents and legal guardians gained the ability to download an eligible child’s official South Carolina Certificate of Immunization, DPH Form 4024, through the SIMON Public Portal.
Daycare families may still need a provider-issued Form 4024
The online SIMON school-certificate feature is designed for qualifying children in 5K through 12th grade. South Carolina DPH says parents and guardians still need to obtain the appropriate DPH Form 4024 from the child’s healthcare provider for daycare entry because daycare requirements differ.
Why SIMON may not match your immunization record
Official failed-match escalation
Allow the South Carolina reporting window before escalating
South Carolina DPH says vaccination records may take up to 10 business days to appear after the vaccine is administered.
Keep the provider or pharmacy administration record while waiting for the normal registry-reporting period.
Check SIMON again later and keep the original proof available for a deadline.
Contact the administering provider and ask it to verify that the dose and patient information were reported correctly.
How to correct a South Carolina immunization record
Start with the organization that can verify the original vaccination. SIMON support can help with access, but the healthcare provider or pharmacy that administered a dose is usually the strongest source for correcting the underlying clinical information.
| Problem | Useful evidence | Best first contact |
|---|---|---|
| Missing vaccine | Provider chart, pharmacy administration record or original vaccine documentation. | Administering provider or pharmacy. |
| Wrong administration date | Visit summary, pharmacy record or original administration record. | Organization that administered the vaccine. |
| Wrong patient name | Correct healthcare demographic record and appropriate identity documentation if requested. | Provider holding the incorrect patient profile. |
| Wrong email or phone | Current contact information. | Provider first; SIMON request route when public access remains blocked. |
| Out-of-state dose absent | Official provider or previous-state registry history. | Current South Carolina healthcare provider for review. |
How to recover vaccinations that may predate SIMON reporting
DPH launched SIMON in 2020, and mandatory provider reporting to the state immunization registry applies from January 1, 2017. Older vaccination histories can therefore require several record sources.
- Current primary-care provider.
- Former pediatrician.
- Hospital records department.
- Previous clinic network.
- Local health department.
- Previous schools.
- College health office.
- Employee health.
- Military medical record.
- VA record when applicable.
- Old vaccination cards.
- Baby books.
- Camp forms.
- Immigration documents.
- Travel-clinic records.
If the old provider closed
Military vaccinations may require a separate record route
DPH states that SIMON does not currently receive immunization information from military facilities in the same way that it receives South Carolina provider data.
- Base medical clinic.
- Military treatment facility.
- Service medical record.
- TRICARE-connected records.
- VA record when applicable.
DPH identifies Form 4375, the Military Certification of South Carolina School and Childcare Immunization Requirements, for qualifying military families.
It can be used with the child’s DD-2766-C or P-0630 vaccination record under the applicable process.
Vaccines from another state may not appear in SIMON
SIMON and DPH HealtheLife serve different purposes
| Portal | Main purpose | How access begins |
|---|---|---|
| SIMON Public Portal | Retrieve available statewide immunization-registry information. | Enter patient/dependent information and complete identity verification. |
| DPH HealtheLife | View records connected with participating South Carolina DPH health-department clinical services, including immunization information. | Request access during a health-department visit and use the invitation sent by email. |
Missing records, Special Exemption and religious exemption are different situations
You are trying to document vaccinations already received. Continue with SIMON, provider, previous school and previous-state records.
A school may issue the South Carolina Certificate of Special Exemption when a qualifying student cannot immediately secure immunizations or documentation.
These follow separate South Carolina exemption rules and are not substitutes for a temporarily missing vaccination record.
30-day Special Exemption
Religious exemption
- Available for qualifying childcare and K–12 situations.
- The parent/guardian/person in loco parentis must complete the official process.
- The form must be notarized.
- DPH provides in-person and online health-department routes.
- South Carolina says religious exemptions under this rule are not used for employers or higher education.
- A child with an applicable exemption may be excluded during a vaccine-preventable-disease outbreak.
Confirm the accepted proof before requesting extra records or testing
| Need | Good starting source | Question to ask |
|---|---|---|
| College | SIMON Form 4025, provider history and student-health portal. | Which vaccines, dates and document formats do you accept? |
| Healthcare employment | SIMON, provider, pharmacy and occupational-health records. | Do you require vaccine dates, laboratory evidence or your own employee-health form? |
| Travel | Provider, pharmacy, SIMON and travel clinic. | Do you require a specific international vaccination certificate rather than a general history? |
| Immigration medical exam | Existing official vaccination records plus civil-surgeon instructions. | Which records should I bring to the exam? |
| Personal file | DPH Form 4025 plus provider/pharmacy backups. | Are all expected vaccinations included? |
Do not use a public-records FOIA request for your personal vaccination history
South Carolina DPH’s FOIA guidance routes people seeking immunization information to the SIMON Public Portal, local health department or immunization provider rather than the general public-records request process.
Who should you contact for South Carolina immunization-record help?
| Problem | Best contact | Prepare before contacting |
|---|---|---|

SC DHEC immunization records FAQs
How do I get SC DHEC immunization records in 2026?
DHEC is the former agency name. Use the current South Carolina Department of Public Health SIMON Public Portal to search for your own or an eligible legal dependent’s available vaccination record.
Do I need a SIMON account to get my personal record?
The resident-facing SIMON Public Portal begins with an Immunization Record Info Request for yourself or a legal dependent. It is different from professional SIMON accounts used by healthcare and other authorized organizations.
What information does SIMON use to find my record?
South Carolina DPH says the portal uses patient first and last name, gender, date of birth and contact information already associated with the patient’s SIMON record. DPH’s FAQ says a matching email is used for two-factor verification.
What is the difference between DPH Form 4024 and DPH Form 4025?
DPH Form 4024 is the South Carolina Certificate of Immunization used for applicable school and childcare attendance. DPH Form 4025 is the personal immunization history and is not a replacement for the school certificate.
Can I download my child’s school Form 4024 online?
Yes, when the child is an eligible 5K–12 student whose SIMON record meets the applicable South Carolina requirements. Parents and legal guardians gained this online option beginning August 1, 2025.
Can I use the online school certificate for daycare?
The current online certificate feature is limited to qualifying 5K–12 students. DPH says families needing a daycare Form 4024 should obtain the appropriate certificate from the child’s healthcare provider.
Why can’t SIMON find my immunization record?
Common reasons include a different name, mismatched email or phone information, a recent vaccination still being reported, vaccines administered before 2017, out-of-state vaccination, military records or provider reporting problems.
How long can a recent vaccine take to appear in SIMON?
South Carolina DPH says vaccination records may take up to 10 business days to appear. If a South Carolina vaccination remains missing after that period, contact the provider that administered it and ask the provider to verify the registry submission.
Do military or out-of-state vaccines always appear in SIMON?
No. DPH says vaccinations administered in other states or military facilities may not be available through SIMON. Contact the original provider, military clinic or registry in the jurisdiction where the vaccination was given.
What if I cannot access SIMON because my email or other information is missing?
Follow the portal error guidance and use the official SIMON Vaccination Record Request described by South Carolina DPH. The SIMON team can address missing information and notify you when you can try the Public Portal again.
