What if the original vaccination record was lost or destroyed by the medical provider?

Delaware’s public-school regulation has a specific process for this narrow situation. It is different from simply being unable to remember a portal password or locate a personal copy.

First confirm that the provider’s record was actually lost or destroyed Search DelVAX, the original provider, successor provider, pharmacy and school records first.
