University of Florida immunization records involve two very different tasks. Incoming students usually need to satisfy UF Health Compliance and clear an immunization hold, while current or former students may instead need a copy of immunization records already maintained by the UF Student Health Care Center.

How to submit University of Florida immunization records

Check ONE.UF first Review your program-specific immunization To-Do Items so you know which requirements actually apply to you.
Open the UF Student Self-Service Portal Go to the Immunizations area and review what UF already has.
Select View Missing Immunizations Do not upload duplicate records until you know which items remain unresolved.
Check Florida SHOTS when you received vaccines in Florida Eligible Florida registry records may already have populated your account.
Use the state-record request for out-of-state vaccinations Select the state or states where vaccines were given and enter the requested historical address information.

UF does not send a separate confirmation that an individual immunization form has been received. The current UF form tells students to monitor their UF account to see whether the checklist has cleared.

Check for Florida SHOTS records before manually uploading

UF says Florida SHOTS information may pre-populate an admitted student’s immunization account about one week after UF acceptance when matching Florida records are available.

Sign in to the Student Self-Service Portal Open Immunizations.
Choose View Missing Immunizations Review whether expected Florida doses are already listed.
Compare each required item with the displayed dates A Florida SHOTS history can still be incomplete.
Check ONE.UF The real goal is compliance clearance, not simply seeing vaccine dates.

Florida SHOTS does not automatically override program requirements. A student can have vaccine dates on file and still have an unresolved program-specific immunization, screening, laboratory or form requirement.

How to request another state’s immunization record through UF

Open the UF Student Self-Service Portal Go to the Immunizations area.
Select the blue record-request icon This starts UF’s state-record request workflow.
Select every state where you received vaccinations Students who moved may need to select more than one state.
Enter the name and address associated with each state Historical addresses can be important for registry identity matching.
Select Finish & Complete and agree to the terms Complete the portal request.
Check the Immunizations page again after several hours UF currently tells students to return later to see whether information has updated.

Multiple-jurisdiction history: One state registry may not contain doses given in another state, another country, a military facility or a federal health system.

What types of immunization records can UF consider?

UF currently lists several possible documentation routes. Health Compliance makes the final determination about whether an alternative record can be used instead of the UF Mandatory Immunization Form.

Completed UF Immunization Form

Have the appropriate healthcare office complete and verify the form required for your academic program.

Official state registry record

Useful when the vaccine history is available from Florida SHOTS or another state’s official registry.

Official college or university record

Useful for transfer or graduate students whose previous institution already maintained verified immunization documentation.

Military vaccine record

UF lists military vaccination records as another possible record source.

Alternative record does not mean automatic acceptance. UF Health Compliance makes the final determination about whether the documentation satisfies the student’s requirement.
General Education

UF General Education immunization requirements

General Education requirement overview

Hepatitis B At least one dose, qualifying immunity evidence or signed UF waiver. Use UF’s current waiver process when declining.

TB screening Required for international General Education students. Testing must fall within the 12 months before starting UF classes.
Do not confuse every field on the form with a matriculation requirement. The current General Education form places Td/Tdap, Varicella, Hepatitis A, HPV, COVID-19 and Meningitis B in an “Optional Immunizations — Not Required for Matriculation” section.

Health Professions & Veterinary Medicine

Program requirements can be significantly different

Clinical programs may add requirements after admission. UF states that additional vaccines or screenings may be required after matriculation for clinical residency or rotation participation.
International students

TB screening documentation has its own failure points

UF’s General Education form says international students must complete TB screening within the 12 months before the start of UF classes. Health Professions students should use their program-specific TB instructions.

BCG vaccination does not satisfy the UF TB-screening requirement. If the required screening is positive, the current General Education form requires a chest X-ray and a copy of that report.
Common processing problem: UF specifically asks students troubleshooting a TST to confirm the skin test was read within 48–72 hours.
Waivers and exemptions

Use UF’s own waiver or exemption process

A UF waiver does not necessarily satisfy every clinical site. Health-related programs, clinical rotations and outside practice sites may have additional rules.
Students under 18

Minor students have extra signature and consent steps

UF’s current General Education form tells students under 18 to include a signed Minor Medical Treatment Consent Form.

Submission troubleshooting

Why UF records get rejected or stay incomplete

Dates were never entered Uploading a document does not replace all required portal data-entry steps.
Document was not uploaded in the same session UF says entered dates are not saved unless the required form is uploaded before logout.
Provider signature or stamp is missing UF requires authorized provider/office verification on the form.
Document is illegible UF says illegible documentation will not be processed.
Document is not in English UF’s current upload guidance requires legible documentation in English.
MMR timing does not satisfy the requirement UF tells students to verify both MMR doses occurred after 12 months of age and meet required dose spacing.
MenACWY date is not qualifying UF tells students to confirm the vaccine was given in 2005 or later and on or after age 16.
Titer report is missing A handwritten titer result is not enough when UF requires the actual lab report.
TB documentation is incomplete Missing placement/read dates, millimeter result, laboratory report or chest X-ray report can delay processing.

What to do when the immunization hold is still showing

Confirm your upload succeeded A submission confirmation tells you the upload step completed.
Allow the published review time UF currently says to allow about one week for verification.
Return to ONE.UF Look for the immunization hold and any new To-Do Item.
Open View Missing Immunizations Compare the outstanding item with what you actually submitted.
Check data and documentation together Verify date entry, dose timing, provider signature/stamp, laboratory report, TB documentation and waiver.
Contact Health Compliance if the status still appears inconsistent Have your UFID, submission date and exact ONE.UF item ready.
Do not wait for an email receipt. UF’s form says Health Compliance does not send confirmation that an individual immunization form has been received. Monitor the UF account instead.

What if Preview or registration is approaching?

UF’s current General Education form tells students to submit missing documentation at least three weeks before orientation or registration.

Records already submitted and processing

Current UF Preview guidance says the immunization hold can be temporarily lifted for Preview registration while submitted records are still processing.

Records not yet submitted

Do not assume Preview’s temporary process will solve a missing submission. Complete the Health Compliance workflow as soon as possible.

Temporary Preview access is not final clearance. UF says all required immunizations still must be submitted and approved before the start of classes.

Need a vaccine instead of a record?

UF says required vaccines are available at SHCC

If the problem is a genuinely missing required vaccination rather than missing documentation, UF says required vaccines are available through the Student Health Care Center and that SHCC accepts most health-insurance plans.

Medical decision: Do not repeat a vaccination simply because a record is missing from one system. Show all available records to a qualified healthcare professional and follow the applicable UF requirement.

UF’s current form directs students to enter vaccine dates and upload the form through the Student Self-Service Portal.

The General Education form warns that ordinary internet email is not necessarily secure and confidentiality cannot be guaranteed.

Never email GatorLink passwords, authentication codes or unnecessary identity documents. Follow the exact document channel provided by the official UF office handling the request.

How to request immunization records already held by UF SHCC

This workflow is for records maintained by the UF Student Health Care Center. It is different from the incoming-student Health Compliance submission process.

Use the SHCC Medical Release / ROI form designated for current students.

Former student or non-student

Use the SHCC records-release form designated for former students or non-students.

Open SHCC Forms & Records Choose the request form matching your current status.
Complete the request in writing UF says requests for medical records, including immunization records, must be written and signed by the patient.
Request only what you need The current student ROI form includes a specific checkbox for Immunization Records.
Specify the recipient Identify whether the records should go to you, another healthcare provider, school, employer or another authorized recipient.

Former UF students

How long does UF Student Health keep medical records?

SHCC currently says UF student medical records are retained for 10 years following enrollment inactivity or graduation. If the student re-enrolls within that period, the record is reactivated.

More than 10 years after UF inactivity? SHCC states that records are destroyed after the 10-year retention period. If the chart is no longer available, rebuild the immunization history using outside sources.

Adult-student privacy

Can a parent obtain an adult UF student’s medical records?

Student age 18 or older

SHCC says it generally may not release information about the student’s medical care to a parent, guardian or other third party without the patient’s express permission.

Patient age 17 or younger

SHCC states that medical records for patients 17 and younger are available to parents/legal guardians under the applicable minor-patient rules.

Compliance status and clinical records are different. Knowing that an adult student’s UF hold exists does not automatically authorize release of that student’s medical chart.

Missing documentation is not proof that a vaccine was never received. Do not repeat a vaccination or order laboratory testing solely because one portal does not display the record. Review the situation with a qualified healthcare professional and the receiving UF program.

ImmunizationRecord.org is an independent informational website. It is not the University of Florida, UF Health, UF Health Compliance, UF Student Health Care Center, ONE.UF, Florida SHOTS, Florida Department of Health, CDC, a healthcare provider, academic program, employer or government registry.

We cannot access your UF account, clear an immunization hold, search Florida SHOTS, approve a waiver, change your record, submit UF documentation or release records held by UF. Do not send this website your UFID, GatorLink password, authentication code, medical records, laboratory reports, photo identification, Social Security number or other sensitive information.
